白血病患者PICC相关性血流感染目标性监测及危险因素分析

【摘要】:目的探讨白血病患者外周中心静脉导管(PICC)相关性血流感染的危险因素。方法选择2015年6月-2016年12月医院收治的白血病PICC置管患者150例,统计PICC相关性血流感染患者临床资料,经单因素及Logistic多因素分析,探讨PICC置管后发生相关性血流感染的危险因素。结果 150例患者中出现导管相关性血流感染20例,感染率13.33%,共检出病原菌20株,其中革兰阴性菌10株占50.00%,以肺炎克雷伯菌和大肠埃希菌为主;革兰阳性菌10株占50.00%,以表皮葡萄球菌为主;多因素logistic回归分析结果显示,年龄≥65岁、外周血象2.0×10~9/L、穿刺次数≥6次、化疗次数≥11次、留置时间≥11个月、伴有糖尿病是PICC导管相关性血流感染的独立危险因素(P0.05)。结论导致白血病患者PICC相关性血流感染的原因较多,应针对危险因素,化疗前应积极处理合并症,选择合理的导管种类、穿刺和封管方式,根据药敏试验科学使用抗菌药物,缩短置管时间,从而减少PICC相关性血流感染风险。
