大潮气量通气致兔VILI模型建构及IL-1β、NF-κBp65表达研究

【摘要】:第一部分 大潮气量通气建立兔的量佳VILI实验动物模型 目的 应用大潮气量机械通气制作兔的最佳VILI实验动物模型。 方法 根据黄金分割法原理,实验组分三组分别采用三种不同潮气量68mL/kg、60mL/kg和45mL/kg各持续通气1分钟造成兔的急性肺损伤,取0h,,12h,24h,48h四个时间点进行观察,统计存活数观察兔存活情况,计算各时间点肺湿/干重比反映肺水肿严重程度及变化,传统病理切片观察各时间点肺组织形态学改变。 结果 1、实验组A(V_T68mL/kg)、实验组B(V_T60mL/kg)和实验组C(V_T45mL/kg)兔48h存活率分别为58.33%(7/12)、91.67%(11/12)和100%(12/12)。2、与正常对照组(4.40±0.02)相比,实验组A(V_T68mL/kg)、实验组B(V_T60mL/kg)和实验组C(V_T45mL/kg)的肺湿/干重比在0小时无明显变化,比值分别为4.46±0.08、4.48±0.08和4.43±0.12:12小时明显增高,比值分别为5.27±0.18、4.84±0.12和4.64±0.06:在24小时时达峰值,比值分别为5.50±0.18、5.29±0.12和4.88±0.07;48小时后降低,比值分别为5.00±0.31、4.774±0.21和4.46±0.10。3、组织病理学切片显示机械通气后不同时间肺组织均有不同程度形态学改变,实验组A(V_T68mL/kg)肺组织形态学改变非常明显;实验组B(V_T60mL/kg)肺组织形态学改变明显;实验组C(V_T45mL/kg)肺组织形态学改变不明显。 结论 大潮气量通气成功建立兔的最佳VILI实验动物模型,建立的指标为V_T60mL/kg、RR50次/分、FiO_221%。
[Abstract]:Part one: establishment of VILI Experimental Model of Rabbit by Tidal Vapor ventilation Objective to establish the best animal model of VILI by mechanical ventilation. Methods according to the principle of golden division, the experimental group was divided into three groups, three different tidal volume of 68 mL / kg 60 mL / kg and 45mL/kg continuously ventilated for 1 minute each to cause acute lung injury in rabbits. The rabbits were observed at four time points (0 h, 12 h, 24 h and 48 h), and the survival rate was observed. Lung wet / dry weight ratio at different time points was calculated to reflect the severity and changes of pulmonary edema. Results 1. The 48h survival rates of the experimental group (AVT 68 mL / kg, the experimental group BVT 60 mL / kg) and the experimental group Cviv T 45 mL / kg) were 58.337 / 121.67 / 1112) and 100 / 12 / 12 / 12, respectively, compared with 4.40 卤0.02 in the normal control group. There was no significant change in the lung wet / dry weight ratios in the experimental group AVT 68mL / kg, the experimental group BVVT 60mL / kg and the experimental group CVT 45mLkg-1 / kg. The ratios were 4.46 卤0.08 卤4.48 卤0.08 and 4.43 卤0.12: 12, respectively. The ratios were 5.27 卤0.184.84 卤0.12 and 4.64 卤0.06, respectively, and reached the peak value at 24 hours. The ratios were 5.50 卤0.185.29 卤0.12 and 4.88 卤0.0748 hours, respectively. The ratios were 5.00 卤0.31 卤4.774 卤0.21 and 4.46 卤0.10.3 respectively. Histopathological sections showed that there were different degrees of morphologic changes in lung tissue at different time after mechanical ventilation. The lung histomorphologic changes of the experimental group were not obvious. Conclusion the best animal model of VILI in rabbits was successfully established by spring tide ventilation, and the established index was: v _ T _ (60 mL / kg) RR 50 times / min Fio _ 221 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1).
