道路交通事故颌面部损伤的法医临床学应用研究

【摘要】: 目的:研究道路交通事故颌面部损伤的流行病学特点和法医临床学损伤特征;分析颌面部损伤的伤情鉴定、伤残评定和后期医疗费用评估特点及相关因素;为现行的伤情及伤残标准等修订完善提供科学依据。方法:应用医学统计学方法对2006年和2007年昆明地区道路交通事故颌面部损伤704例法医临床鉴定案例进行系统性分析研究。结果:本组704例道路交通事故颌面部损伤的案例中,伤情鉴定290例(41%),伤残评定154例(22%),后期医疗费用评估260例(37%),受伤人群的流行病学特点为:男性多,青壮年多,已婚多,工人和农民多。(1)伤情鉴定:290例中重伤13例(4.5%),轻伤163例(56.2%),轻微伤114例(39.3%)。受伤人员中,乘客比例最多(40.7%),但驾驶员和副驾驶位人员损伤最重;在一个月以内鉴定的例数最多;损伤多为颌面部软组织损伤,其次为颌面骨折,损伤程度以轻伤为主。(2)伤残评定:154例中构成伤残89例(57.8%),最高等级V级,总体分布以Ⅸ级、Ⅹ级轻度伤残为主。构成伤残的以驾驶员居多,其次为副驾驶位人员,伤残级别较高的均发生于驾驶员和副驾驶位人员;以伤后3-4月进行伤残评定的例数最多;损伤特征主要为瘢痕及色素沉着,其次为牙齿脱落。(3)后期医疗费用评估:260例后期医疗费用评估结果主要集中在2000~4000元。治疗项目主要为瘢痕治疗;瘢痕与色素的治疗费用多集中在4000元以内:伤情大多以轻伤为主,与后期所需治疗费用存在正相关性;是否需要二次手术及有无并发症与后期所需治疗费用呈正相关。结论:道路交通事故受伤人群以男性、中青年、已婚、农民为主,致伤致残率高。驾驶员与副驾驶位人员受伤程度较重。伤情鉴定中以颌面部软组织损伤最为常见,损伤程度以轻伤为主;伤残程度以轻度伤残为主;后期医疗费用评估以2000~4000元所占比例最多。伤情鉴定、伤残评定及后期医疗费用评估与受伤人员的年龄、职业、婚姻情况及肤色等社会因素相关性较大。
[Abstract]:Objective: to study the epidemiological characteristics and forensic clinical injury characteristics of maxillofacial injury in road traffic accidents, to analyze the characteristics and related factors of maxillofacial injury assessment, disability assessment and later medical cost assessment, and to provide scientific basis for the revision and improvement of current injury and disability standards. Methods: 704 cases of maxillofacial injury caused by road traffic accidents in Kunming in 2006 and 2007 were systematically analyzed by medical statistics. Results: among 704 cases of maxillofacial injury caused by road traffic accidents, 290 cases (41%) were identified as injury condition, 154 cases (22%) as disability assessment, 260 cases (37%) as late medical cost assessment. (3) later medical cost evaluation: 260 cases of late medical cost evaluation results were mainly concentrated in 2000 / 4000 yuan. The treatment items were mainly scar treatment, and the treatment cost of scar and pigment was mainly less than 4000 yuan: most of the injuries were mainly mild injuries, which were positively correlated with the cost of treatment in the later stage, and whether the need for secondary surgery and complications was positively correlated with the cost of treatment in the later stage. Conclusion: male, young and middle-aged, married and farmers are the main injured population in road traffic accidents, and the disability rate is high. The driver and the copilot were seriously injured.
