老年癌初患者心理焦虑的个案工作实践研究

【摘要】：截止2016年初,我国60周岁及以上人口占13.26%,与2000年人口普查数据相比增加了2.93%。随着出生率降低、人们的预期寿命的延长,我国老龄化问题随之而来。有关医学统计分析表明60周岁及以上人口是癌症高发病率人群。目前中国老年癌症患者正在呈逐年递增趋势。由于年龄的增长和身体机能的不断衰退,老年癌初患者的心理上更加脆弱。因癌症的不可逆性,国内外面对老年癌初患者最普遍的方法是医学治疗和临床照顾相结合。近年来以心理学兴起为背景的心理治疗和临床呵护在缓解老年癌初患者心理焦虑的服务中不断进步。借鉴前人在缓解老年癌初患者心理焦虑的理论基础和实践经验,同时结合笔者在吉大一院放疗科实习的相关经历,笔者试图从社会工作的角度,把老年社会工作的相关理论和社会工作的方法相结合,把理性情绪疗法用于缓解老年癌初患者心理焦虑的个案工作中去,探索社会工作在其中的实践意义。笔者试图以系统理论着眼案主整体情况,运用理性情绪疗法缓解案主的焦虑情绪,以此来检测社会工作在老年癌初患者心理焦虑服务中的作用。在项目进行阶段,笔者以系统视角理论作为开展个案工作的主要理论,运用理性情绪疗法和个案工作方法相融合的社工形式,通过8次个案服务面对面和案主进行交流,同时联系案主家庭相关成员,了解其社会关系网络,引导案主发现问题并共同制定服务计划和所要达到的服务目标,和案主亲人共同见证案主的成长与进步。个案服务结束后,笔者通过焦虑自评量表(SAS)来对比案主的心理变化并根据其家人的交流和督导的评价来获得服务的最后结果。以理性情绪疗法来缓解老年癌初患者心理焦虑,通过面对面的沟通引导案主发现问题,培养案主发现问题和解决问题的能力,发掘服务对象自身潜能,整合案主相关资源来解决服务对象的焦虑情绪,达到案主外在行为的改变。
[Abstract]:By the beginning of 2016, the population aged 60 and above accounted for 13.26 percent, an increase of 2.93 percent compared with the 2000 census data.With the decrease of birth rate and the prolongation of life expectancy, the aging problem of our country follows.Medical statistics and analysis show that the population aged 60 and above is a high incidence of cancer.At present, the elderly cancer patients in China are increasing year by year.As a result of the age and the body function receding, the old age cancer early patient psychologically is more vulnerable.Because of the irreversibility of cancer, medical treatment and clinical care are the most common methods to face the aged patients with cancer at home and abroad.In recent years, psychotherapy and clinical care, with the background of the rise of psychology, have made continuous progress in the service of relieving the psychological anxiety of the aged patients at the beginning of cancer.Referring to the theoretical basis and practical experience of relieving the psychological anxiety of the aged patients at the beginning of cancer, and combining with the relevant experience of the author's practice in the radiotherapy department of the first Hospital of Jida, the author tries to make an attempt from the angle of social work.Combining the theory of social work with the method of social work, the rational emotional therapy is used to alleviate the psychological anxiety of the elderly patients with cancer, and the practical significance of social work is explored.The author attempts to use the system theory to focus on the overall situation of the victim and to use rational emotional therapy to alleviate the anxiety of the victim so as to examine the role of social work in the psychological anxiety service of the elderly patients with cancer at the beginning of the year.In the stage of the project, the author takes the theory of system perspective as the main theory to carry out the case work, using the social worker form of rational emotional therapy and the method of case work, through 8 times of case service face-to-face and the case owner to communicate.At the same time, contact the relative members of the victim's family, understand its social network, guide the owner to find out the problem and make the service plan and the service goal to be achieved together, and the relatives of the victim witness the growth and progress of the victim.After the case service, the author compares the psychological changes of the victim with SAS and obtains the final results according to the communication and supervision of his family.Rational emotional therapy was used to relieve the psychological anxiety of the aged patients at the beginning of cancer, to guide the victim to discover problems through face-to-face communication, to cultivate the ability of finding problems and solving problems, and to explore the potential of the clients themselves.Integrate the relevant resources to solve the anxiety of the client and achieve the change of the external behavior of the client.
