老年照料对家庭照料者医疗服务利用的影响——基于中国健康与养老追踪调查数据的经验研究

【摘要】：本文利用2011年和2013年中国健康与养老追踪调查(CHARLS)数据,运用固定效应模型和倾向得分匹配法,考察了老年照料对女性家庭照料者医疗服务利用的影响。结果发现,总体上女性家庭照料者的门诊利用概率增加了5.18%~6.95%,但不同年龄组存在异质性;低龄老年照料者(56~65岁)的门诊利用概率增加了10.79%~12.90%,其医疗负担有可能增加;而中年照料者(45~55岁)的门诊利用概率不变甚至下降,这主要是由于照料受时间约束而无暇就医,从而不能及时补偿其健康损耗。上述结论表明,中国家庭老年照料对医疗服务体系提出了挑战,本文从照料者的角度提出了增强医疗服务可及性、维护健康的政策建议。
[Abstract]:Based on the CHARLS data of China Health and Old-age Care Survey in 2011 and 2013, the effects of elderly care on the utilization of medical services for female family carers were investigated by using fixed effect model and tendency score matching method.The results showed that the outpatient utilization probability of female family carers increased by 5.18 / 6.95, but there was heterogeneity among different age groups, and the outpatient utilization probability of the young aged carers (56 ~ 65 years old) increased by 10.790.12.90, and the medical burden was likely to increase.The outpatient utilization probability of middle aged carers (45 ~ 55 years old) remains unchanged or even decreases, which is mainly due to the time constraint of care and the lack of time to seek medical treatment, which can not compensate for their health loss in time.The above conclusions indicate that the family care for the elderly in China has posed a challenge to the medical service system. From the perspective of the carers, this paper puts forward some policy recommendations to enhance the accessibility of medical services and maintain health.
