【摘要】:目的:对接受住院诊治的膀胱癌患者人格特征与生命质量状况进行调查研究,了解膀胱癌患者目前生命质量状况,分析不同人口学变量的膀胱癌患者生命质量之间差异以及探索膀胱癌患者人格特征与生命质量之间关系,从而进一步探索出改善与提升膀胱癌患者生命质量提供具有现实意义的理论支撑。方法:本次采用横断面调查方式,选择2015年9月~2016年4月于大连某三级甲等医院接受住院诊治的153例膀胱癌患者纳入研究,使用自制的一般情况问卷、艾森克人格问卷简式量表中国版(EPQ-RSC)、世界卫生组织生存质量测定量表简表(WHOQOL-BREF)对患者进行研究,使用SPSS17.0对数据进行差异性检验、相关和回归分析,以P0.05为统计学显著性差异标准。结果:(1)t检验:膀胱癌患者各维度生命质量的得分由高到低依次是环境维度(12.57±1.6)、生理维度(11.84±1.81)、社会关系维度(11.67±2.08)、心理维度(11.57±1.60)。对膀胱癌患者生命质量各维度与常模进行比较发现,膀胱癌患者的生命质量除环境维度稍高于常模外(t=3.14,p=0.002),其它维度(生理、心理、社会关系)均低于国内常模,且均具有统计学差异(t=-22.25,t=-17.98,t=-13.69,p0.001)。(2)方差分析:利用方差分析对膀胱癌患者人口学变量进行差异性检验,结果得到,不同年龄(F=15.55,P0.05)与不同文化水平(F=38.71,P0.05)的膀胱癌患者的生命质量之间存在差异,而不同职业(F=1.97,p0.05)与不同医疗付费方式(F=0.54,p0.05)的膀胱癌患者的生命质量无统计学差异。(3)相关性分析:对膀胱癌患者的人格特征和生命质量进行相关分析,结果显示,人格特征中神经质和膀胱癌患者总生命质量、生理、心理及环境维度呈负相关(r=-0.349,r=-0.230,r=-0.333,r=-0.242,p0.001);精神质与患者总生命质量、生理及社会关系维度呈负相关(r=-0.287,r=-0.239,r=-0.251,p0.001);内外向与总生命质量、生理、心理及社会关系维度呈正相关(r=0.309,r=0.211,r=0.220,r=0.255,p0.001);掩饰维度与患者总生命质量及心理维度呈正相关(r=0.170,r=0.163,p0.05)。采用逐步回归分析方法进一步探索膀胱癌患者人格特征对膀胱癌患者生命质量的影响,结果得到人格特征中3个维度均进入方程,其中影响大小依次为神经质(b=-0.234,P=0.019)、内外向(b=0.230,P=0.002)及精神质(b=-0.184,P=0.004)。(4)对膀胱癌患者影响因素分析结果得到,文化程度、年龄、精神质3个变量进入回归方程(b=0.595,b=-0.227,b=-0.185,P0.05)。结论:(1)目前,膀胱癌患者整体生命质量状况相对较差。(2)膀胱癌患者的生命质量与患者的年龄以及文化程度有关,与患者的职业及医疗付费方式无关。(3)具有神经质倾向以及精神质倾向的膀胱癌患者与其生命质量呈现负相关关系,且对生命质量的作用显著,具有外向型倾向的膀胱癌患者与其生命质量呈现正相关关系,且对生命质量的作用显著。此外,精神质、年龄、文化程度对患者的生命质量起到较大作用。
[Abstract]:Objective: To investigate the diagnosis and treatment of bladder cancer hospitalized patients personality traits and quality of life of patients with bladder cancer currently understand the quality of life in patients with bladder cancer, analysis between the quality of life of different demographic variables difference and explore the personality characteristics of patients with bladder cancer and the relationship between the quality of life, so as to further explore and improve to provide theoretical support with practical significance to improve the quality of life of patients with bladder cancer. Methods: a cross-sectional survey, study in 153 cases of bladder cancer in September 2015 ~2016 year in April in Dalian some three grade hospital for inpatient treatment, using the general questionnaire, Eysenck personality questionnaire China Version (EPQ-RSC), the self-made scale to determine the quality of survival of WHO (WHOQOL-BREF) study on patients, the data difference test, correlation and regression analysis using SPSS17.0, using P0.05 as a statistically significant difference in standard. Results: (1) t test: the scores of quality of life of all bladder cancer patients ranged from high to low, followed by environmental dimension (12.57 + 1.6), physiological dimension (11.84 + 1.81), social relationship dimension (11.67 + 2.08), psychological dimension (11.57 + 1.60). The various dimensions of quality of life of patients with bladder cancer compared with the norm, the quality of life of patients with bladder cancer in the environmental dimension is slightly higher than the norm (t=3.14, p=0.002), and other dimensions (physical, psychological, social relations) were lower than the norm, and there was statistically significant difference (t=-22.25, t=-17.98, t=-13.69. P0.001). (2) analysis of variance: the variance analysis of difference test of bladder cancer patients, demographic variables results, different ages (F=15.55, P0.05) with different culture levels (F=38.71, P0.05) the difference between the quality of life of patients with bladder cancer, and different occupation (F=1.97, P0.05) with different means of payment (F=0.54, P0.05). There were no significant differences in the quality of life of patients with bladder cancer. (3) correlation analysis: correlation analysis of patients with bladder cancer personality characteristics and life quality showed that the personality characteristics of neuroticism and total life of patients with bladder cancer quality, physiological and psychological and environmental dimensions were negatively correlated (r=-0.349, r=-0.230, r=-0.333, r=-0.242, p0.001); psychoticism was negatively correlated with the total patients the quality of life, physical and social relationship dimension (r=-0.287, r=-0.239, r=-0.251, p0.001); and related to the overall quality of life, physical, psychological and social relationship dimension positively (r=0.309, r=0.211, r=0.220, r= 0.255, p0.001); cover dimensions and total life quality of patients with positive psychological dimension and correlation (r=0.170, r=0.163, P0.05). By stepwise regression analysis to further explore the influence of personality characteristics of patients with bladder cancer on quality of life in patients with bladder cancer, the personality characteristics of the 3 dimensions were entered into the equation, the influence order of neuroticism (b=-0.234, P=0.019) to (b=0.230, P, and =0.002) and psychoticism (b=-0.184, P=0.004). (4) the analysis of influencing factors of bladder cancer patients showed that 3 variables of cultural degree, age and mental quality entered the regression equation (b=0.595, b=-0.227, b=-0.185, P0.05). Conclusion: (1) at present, the overall quality of life of bladder cancer patients is relatively poor. (2) the quality of life of the patients with bladder cancer is related to the age and educational level of the patients, and has nothing to do with the patient's occupation and pay for medical treatment. (3) neuroticism and psychoticism of bladder cancer patients and their life quality is negatively related, and the quality of life significantly, with extroversion tendency of bladder cancer patients and their quality of life was positively correlated, and the quality of life significantly. In addition, mental quality, age, and educational level play a significant role in the quality of life of the patients.
