温阳固肾方治疗糖尿病肾病大量蛋白尿的临床观察

【摘要】：目的:运用温阳固肾方治疗糖尿病肾病,观察糖尿病肾病患者蛋白尿的变化,判断温阳固肾方治疗糖尿病肾病的临床疗效。方法:选取符合纳入标准的患者40例,中医辨证属类型于脾肾阳虚瘀血内阻型。在西医一般治疗和对症治疗的基础上,给予口服温阳固肾方中药汤剂治疗,连续服用2个月后,采用自身治疗前后对照法,观察治疗前与治疗后临床疗效及中医证候积分的变化,比较治疗前与治疗后相关实验室指标的改变。运用SPSS19.0软件对观察指标数据进行统计分析,以此来评估温阳固肾方治疗糖尿病肾病大量蛋白尿的临床疗效。结果:(1)温阳固肾方治疗糖尿病肾病的总有效率为85%。(2)治疗后,中医证候疗效总有效率为85%;中医证候总积分与治疗前相比较有所减少,差异有统计学意义(P0.05)。(3)治疗后与治疗前比较,24小时尿蛋白定量减少,血清白蛋白(serum albumin,ALB)增加,差异有统计学意义(P0.05),治疗后优于治疗前。结论:1、温阳固肾方能降低糖尿病肾病患者24小时尿蛋白定量。2、温阳固肾方能提高糖尿病肾病患者血清白蛋白水平。3、温阳固肾方能明显改善脾肾阳虚瘀血内阻型糖尿病肾病患者的临床症状。
[Abstract]:Objective: To observe the changes of proteinuria in patients with diabetic nephropathy by Warming Yang and consolidating kidney, and to evaluate the clinical effect of Wenyang Gu Shen Recipe on diabetic nephropathy. Methods: 40 patients were selected in accordance with the inclusion criteria. The type of TCM syndrome differentiation was type in the type of spleen and kidney yang deficiency and blood stasis. Based on the western medicine general treatment and symptomatic treatment, given oral Wenyang Gushen Decoction decoction, taking 2 consecutive months after the comparison before and after treatment, before treatment and changes of clinical efficacy and TCM syndrome score after treatment compared with before treatment, after treatment and related laboratory indicators of change. The SPSS19.0 software was used to analyze the observation index data to evaluate the clinical efficacy of Wen Yang Gu Shen prescription in the treatment of diabetic nephropathy with massive proteinuria. Results: (1) the total effective rate of Wenyang solid kidney treatment for diabetic nephropathy was 85%. (2) after treatment, the total effective rate of TCM syndrome was 85%, and the total score of TCM syndrome was less than that before treatment, and the difference was statistically significant (P0.05). (3) after treatment, the 24 hour urine protein level decreased and the serum serum albumin (ALB) increased. The difference was statistically significant (P0.05). After treatment, it was better than that before treatment. Conclusion: 1. Wenyang solid kidney can reduce the 24 hour urine protein in patients with diabetic nephropathy. 2. Wen Yang Gu kidney can improve the serum albumin level of diabetic nephropathy patients. 3. Wenyang Gu kidney can obviously improve the clinical symptoms of diabetic nephropathy patients with spleen kidney yang deficiency and blood stasis type of diabetic nephropathy.
