【摘要】:目的:评价微血管密度(microvessel density,MVD)、超声造影参数、血流动力学参数在前列腺癌(prostatic cancer,PCa)与慢性前列腺炎(chronic prostatitis,CP)中的差异及相关性,为PCa与CP的诊断及鉴别诊断、PCa的预后提供帮助。方法:对广西医科大学第一附属医院2014-2015年泌尿外科收治入选的前列腺病患68例行前列腺经直肠超声(Transrectal Ultrasonography,TRUS),获取血流动力学参数;前列腺经直肠超声造影(contrast enhanced transrectal ultrasound,CETRUS),获取造影参数;超声引导下穿刺活检或手术明确病理类型及格里森(Gleason)评分;进行免疫组化MVD的测定。比较PCa组与CP组、PCa中Gleason评分≤3+4组与Gleason评分≥4+3组的血流动力学、MVD、超声造影参数的差异;研究PCa中MVD与血流动力学参数、超声造影参数的相关性。结果:病理证实前列腺癌患者44例,CP患者24例。PCa组与CP组中的收缩期峰值血流速度(Velocity of flow in systolic,Vs)、阻力指数(Resistance Index,RI)、初始强度(Basic intensity,B)、曲线下面积(Area under the curve,AUC)、峰值强度(Peak Intensity,PI)、MVD差异均有统计学意义(P0.05);PCa中Gleason评分≥4+3组28例,Gleason评分≤3+4组16例,两组间Vs、RI、MVD差异有统计学意义(P0.05);PCa中Vs、RI、AUC与MVD具有正相关(P0.05),r分别为0.578、0.688、0.737。结论:血流动力学、超声造影参数、微血管密度为PCa与CP的诊断与鉴别诊断提供帮助;前列腺癌中超声造影、血流动力学参数可较好的反映微血管密度;血流动力学参数、微血管密度对PCa患者预后的判断有一定价值。
[Abstract]:Objective: to evaluate the microvessel density (MVDV) of microvessel density (MVD), and to evaluate the parameters of contrast-enhanced ultrasound (CEUs).
