医疗机构医用耗材采购管理研究

【摘要】:研究目的本研究拟通过文献研究梳理目前医用耗材采购管理相关政策,通过实地调研和问卷调查掌握我国医疗机构医用耗材采购管理现状,分析目前医疗机构医用耗材采购管理存在的问题,提出规范医疗机构医用耗材采购管理的建议。研究方法(1)文献研究法:通过文献研究,初步了解与此相关的基本知识,了解国内外研究现状,为后期研究奠定基础。通过查阅文献,了解相关研究的主要成果、现状及不足之处,明确本课题重点研究的问题:(2)调查研究法:通过实地调研、专家访谈等方式了解各级医疗机构医用耗材采购内部管理的现状,分析其中存在的问题,探寻问题产生的原因;(3)问卷调查法:通过问卷填写的形式,分析目前医疗机构在医用耗材采购管理方面普遍存在的共性问题及原因,并试图找到初步解决方案。研究结果医用耗材的特点有品种规格多且复杂、医用耗材产品更新换代快、不同品种价值差异很大、安全性要求高、临床使用突然性和多样性、进口产品占据国内高值医用耗材市场的垄断地位等;采购方式主要包括招标采购、竞争性商务谈判、市场摸底谈判;目前我国医疗机构医用耗材管理主要存在以下几方面的问题:品种分类方面,分类方法过于多元化、分类标准不统一、高值耗材与低值耗材界定标准不统一;目录管理方面,没有统一的规范目录、没用统一编码、目录制定方法不统一;准入管理方面,准入方式不统 、准入审核内容不一致、准入权限管理较混乱;采购方式方面,采购方式过于多元化、采购计划较为欠缺、采购频次不合理;配存管理方面,配送企业数量较多、库存量管理不科学、领用后监管不力;信息系统方面,信息化程度低、数据分析功能欠缺、交互方式落后。研究结论对医疗机构医用耗材的品种分类、目录管理、准入管理、采购方式、配存管理和信息系统等六个方面进行深入的研究和论证,为构建能基本在全国范围内普遍适用的医疗机构医用耗材采购管理新模式提出建议。品种分类方面,应当逐步建立以医用耗材标准化分类编码为基础的医用耗材信息化管理制度和系统;目录遴选管理方面,应根据各类品种的质量技术和价格信息综合评价后进行选择,在保证产品质量的前提下,优先选择性价比高的品种;准入管理方面,加强对准入资质的管理、审核;采购管理方面,严格按照采购要求和采购计划,继续推进集中采购工作,对于未纳入所属行政区域省级或市级集中采购范围的,或在集中采购范围内但未有品种中标的,医疗机构可视情况进行招标采购、询价采购、备案采购等特殊方式进行采购;配存管理方面,包括对验收入库管理和出库管理的要求;信息系统管理方面,应采用信息化技术手段,实现对医用耗材采购的全流程、全方位监管。
[Abstract]:The purpose of this study is to sort out the relevant policies of medical consumables procurement management through literature research, and to grasp the current situation of medical consumables procurement management in medical institutions through field investigation and questionnaire survey. This paper analyzes the problems existing in the procurement management of medical consumables in medical institutions, and puts forward some suggestions for standardizing the procurement management of medical consumables in medical institutions. Understand the domestic and foreign research status, lay the foundation for the later research. Through consulting the literature, understand the main results, current situation and shortcomings of the related research, make clear the research method of the key research question of this topic: through the field investigation, Experts interview and other ways to understand the status of medical institutions at all levels of internal management of medical supplies procurement, analysis of the existing problems, to explore the causes of the problems / 3) questionnaire survey method: through the form of questionnaire filling, This paper analyzes the common problems and reasons of medical institutions in purchasing and management of medical consumables, and tries to find a preliminary solution. The value of different varieties is very different, the requirement of safety is high, the clinical use is sudden and diverse, the imported products occupy the monopoly position of the domestic high value medical consumables market, the purchasing methods mainly include bidding and purchasing, competitive business negotiation, etc. At present, there are some problems in the management of medical consumables in medical institutions in our country: the classification methods are too diversified, the classification standards are not uniform, and the definition standards of high value consumables and low value consumables are not unified. In the area of directory management, there is no unified standard catalogue, no unified coding, and no unified method for cataloguing. In access management, access methods are not uniform, the contents of access review are inconsistent, and the management of access rights is relatively confused. In terms of procurement methods, The procurement mode is too diversified, the purchase plan is relatively deficient, the procurement frequency is unreasonable; in the aspect of inventory management, the number of distribution enterprises is more, the inventory management is not scientific, the supervision after receiving the goods is weak; the information system aspect, the informationization degree is low, The function of data analysis is deficient, and the interactive mode is backward. Conclusion the six aspects of medical consumables classification, catalogue management, access management, purchasing method, storage management and information system are deeply studied and demonstrated. In order to set up a new management model of medical consumables procurement and management, which can be widely used in the whole country, some suggestions are put forward. The information management system and system of medical consumables based on standardized classification and coding of medical consumables should be gradually established. In the selection and management of catalogues, the selection should be based on the comprehensive evaluation of the quality, technology and price information of various varieties. Under the premise of ensuring product quality, priority should be given to the selection of varieties with high performance-to-price ratio; access management should strengthen the management and examination of entry qualifications; and procurement management should continue to promote centralized procurement in strict accordance with procurement requirements and procurement plans. For those not included in the scope of centralized procurement at the provincial or municipal level in the administrative region, or in the scope of centralized procurement but not in the bid of a variety, the medical institution may, according to the circumstances, conduct the procurement by bidding, asking for price, filing for purchase, and other special means; In the aspect of inventory management, including the requirement of acceptance and check in and out of storage, and the information system management, information technology should be adopted to realize the whole process of purchasing medical consumables, and to supervise the whole management of medical consumables in all directions.
