张子和攻邪论学术思想研究

【摘要】：张子和是中国医学史上一位卓尔不群的杰出医家,金元四大家之一,为"攻邪派"的代表。目前对张子和学术思想的研究挖掘不够精深,对其病因、病机、辨治机理认识未作深入系统探讨,对攻邪论学术思想仍然缺乏全面客观评价。张子和攻邪论学术思想尚待系统整理阐述,需要分析归纳其内涵机理。通过对张子和学术理论和应用进行整理研究,归纳辨治机理,总结方药特色,阐述后世对张子和学术评价和传承发展情况,以对进一步研究、发扬张子和学术思想有所助益。研究方法采用传统文献研究方法、比较研究法、文献统计法。通过对张子和攻邪论学术思想有重要影响的经典著作与医家学术思想溯源,探讨张子和对虚实攻补的继承与发展情况。将疾病按病因分为"始因气动而内有所成者"、"始因气动而外有所成者"、"不因气动而病生于内"、"不因气动而病生于外"四类,整理张子和医理、医案,归纳其辨治机理,对比后世医家对疾病病因、病机、治法观点。整理张子和妇、儿、五官科证治特色。归纳张子和攻补理法内涵。对《儒门事亲》医案所载方药进行全面归纳整理,使内容更为完整与条理。对六门制方用药特色进行分析。整理后世对张子和学术发展情况,客观认识张子和学术思想对后世的影响。张子和攻邪论学术思想将《内经》、《难经》理论作为阐释病证及治疗方法重要依据,善用张仲景治法方药,尊崇刘完素学术思想。张子和将疾病按病因分为四类,"始因气动"有病发于内、外之分,"不因气动"有内因、外因之分,通过病案理法方药分析,具体理解攻邪论学术思想的临证运用。后世诊治风痹痿厥、九气为病、疝病、暴泻诸病多遵张子和所论。张子和治疗妇、儿、五官科疾病以清热利水、和血调经为要。张子和攻补理法内涵丰富,以汗、吐、下三法为纲,能兼众法。攻法包括汗、吐、下三法,发表、攻里祛邪治法,疾病病机从火和湿认识治以清泻火热、淡渗利湿、辛通润燥,五郁(风木郁、暑火郁、湿土郁、燥金郁、寒水郁)为病治以汗法、吐法、下法、利小便、折冲逆。补法包括祛邪即是补正、养生以食补、权衡平补、五味以补脏腑。调治法包括以水调诸药的经验,调治阴阳、调养胃气、通经调血、随证调治。整理补充按三法六门、"兼治"、"独治"、"调治"分类的方剂,根据四气五味分类三法用药,统计六门方剂用药频次。后世医家对张子和学术评价褒贬不一,但在临证诊治中攻邪祛病为重要的治疗方法,不离广义的汗、吐、下三法,随着中医学发展进一步细化病因、病机的认识,治疗缓和并具有针对性,补充疾病的虚寒证治。
