【摘要】：目的:制订确切疗效、方便应用、科学规范的穴位敷贴干预小儿哮喘中医治未病实践指南,并提供中医指南制修订的技术方法。方法:采用文献研究、三轮专家问卷调查(Delphi法)、专家论证会、同行征求意见、临床评价(方法学质量评价、临床一致性评价)、专家指导组审核相结合的方法制订"中医治未病实践指南穴位敷贴干预小儿哮喘"。文献研究:遵循循证医学的原则,以网络检索为主结合手工检索查询符合质量要求的文献作为指南的证据,经专家共识法形成指南的推荐建议,汇总、归纳后形成"中医治未病实践指南穴位敷贴干预小儿哮喘"的文献研究总结。专家问卷调查(Delphi法):基于文献研究总结,设计第一轮专家调查问卷,以邮件形式邮寄给全国遴选出的40位专家,根据问卷回馈的专家评分进行总结分析形成下一轮问卷内容,再次邮寄给回复积极的专家进行第二、三轮问卷调查,经统计分析后形成"中医治未病实践指南穴位敷贴干预小儿哮喘"的初稿。专家论证会:对于指南初稿形成过程中存在需要专家面对面讨论分析的问题,项目工作组邀请12名中医儿科、中西医结合儿科、文献研究等的专家召开专家论证会,专家讨论后对相关问题的解决建议基本达成一致,形成"中医治未病实践指南穴位敷贴干预小儿哮喘"的征求意见稿。同行征求意见:为了使指南内容更完善,项目工作组向标准化相关单位的专家发送指南征求意见稿及征求意见处理表以征求指南修改意见,经项目工作组讨论分析修改后,形成指南的评价稿。临床评价(方法学质量评价、临床一致性评价):方法学质量评价是邀请包括中医儿科、文献学等专业的4位专家,应用AGREEⅡ工具对《中医治未病实践指南穴位敷贴干预小儿哮喘》评价稿进行方法学的质量评价;临床一致性评价是在专家指导组指导下,选取不同地域的9个医疗机构对《中医治未病实践指南穴位敷贴干预小儿哮喘》评价稿与临床实践进行一致性评价,形成《中医治未病实践指南穴位敷贴干预小儿哮喘》的审核稿。专家指导组审核:项目工作组将指南审核稿提交中医治未病标准制修订"儿童人群"专家指导组进行审核,并提出修改意见,进一步完善"穴位敷贴干预小儿哮喘中医治未病实践指南",形成指南的送审稿。结果:通过文献研究、专家问卷调查(Delphi法)、专家论证会、同行征求意见、临床评价(方法学质量评价、临床一致性评价)、专家指导组审核相结合的方法,制订出方便应用、科学规范的中医治未病实践指南,临床一致性评价结果为本指南与临床实际操作符合度较高,证明本指南制订的技术方法适合中医指南的制修订工作。
[Abstract]:Objective: to establish a scientific and standard guidance for the treatment of children asthma by acupoint application, and to provide the technical methods for the revision of TCM guidelines. Methods: literature study was used. Three rounds of expert questionnaire survey, Delphi method, expert demonstration meeting, peer consultation, clinical evaluation (methodological quality evaluation, Clinical consistency evaluation, expert guidance group review combined with the method of developing "the practice guide of traditional Chinese medicine for preventing disease intervention in children asthma". Literature study: follow the principle of evidence-based medicine, With the combination of online retrieval and manual retrieval of documents that meet the quality requirements, as evidence of the guide, the expert consensus method forms the recommended recommendations of the guidelines and summarizes them. After summing up, it formed the literature research summary of "the practice Guide of TCM treatment and Predisease intervention in Children Asthma". The first round of expert questionnaire was designed based on the summary of literature research. Mail it to 40 experts selected throughout the country, summarize and analyze the next round of questionnaires according to the feedback expert scores, and mail the second and third rounds of questionnaires to the responding active experts again. After statistical analysis, a first draft of the "practical guidelines for the practice of traditional Chinese Medicine in treating and preventing Diseases in Children with Asthma" was formed. The expert demonstration meeting: there are problems that need experts to discuss and analyze face to face in the course of the formation of the first draft of the guide. The project working group invited 12 experts from pediatrics of traditional Chinese medicine, integrated traditional Chinese medicine and western medicine, literature research and other experts to convene an expert demonstration meeting. After discussion, the experts basically reached agreement on how to solve the related problems.
