新疆维吾尔族中老年男性SHBG水平及其相关因素分析

【摘要】:目的:探讨新疆维吾尔族中老年男性性激素结合球蛋白(SHBG)水平变化及与其影响因素的相关性。为临床运用中监测血清SHBG提供更多的指导意义。方法:采用整群随机抽样的方法,选择新疆维吾尔自治区乌鲁木齐市天山区二道桥片区及新疆托克逊县3镇4乡居民进行横断面调查,收录了2015年9月-12月共620例45岁以上维吾尔族男性的基本资料包括:身高、体重、腰围、血压,实验室检查结果包括:性激素结合球蛋白(SHBG)、睾酮(TT)、空腹血糖(FBG)、OGTT2h血糖(PBG)、糖化血红蛋白(HbAlC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、胆固醇(TC)、血尿酸(UA)、空腹胰岛素(Ins)、维生素D(VitD)、骨钙素(OST)。并通过公式计算体重指数(BMI)、胰岛素抵抗指数(HOMA-IR)。按照血清SHBG水平分为低水平及正常水平分两组,比较两组之间上述指标水平,并探讨SHBG与上述指标之间的关系。结果:维吾尔族中老年男性低水平性激素结合球蛋白组的TT、Ins、HOMA-IR、LDL-C水平均高于性激素结合球蛋白正常组,而年龄、收缩压、VitD水平则低于SHBG正常组,P均小于0.05。应用Spearman秩相关作相关分析,维吾尔族中老年男性年龄、收缩压、舒张压、BMI、HDL-C、VitD、OST均与SHBG呈正相关(r=0.347,r=0.158,r=0.125,r=0.097,r=0.102,r=0.439,r=0.116);FBG、TG、LDL-C、TT、Ins、HOMA-IR均与SHBG呈负相关(r=0.167,r=0.088,r=-0.424,r=-0.168,r=-0.725,r=-0.718),P均小于0.05。回归分析提示低SHBG水平保护因素为年龄、HDL-C、VitD,而FBG、LDL-C、HOMA-IR、Ins、TT则为其危险因素。结论:对于维吾尔族中老年男性来说,年龄、HDL-C、VitD可能为低水平SHBG的独立保护因素。
[Abstract]:Objective: To investigate the Xinjiang Uygur elderly male sex hormone binding globulin (SHBG) levels and its influence factor relevance. In order to provide more guidance for monitoring serum SHBG clinical application. Methods: using cluster random sampling method, a cross-sectional survey was conducted 4 rural residents choose the Xinjiang Uygur Autonomous Region Erdaoqiao Urumqi Tianshan District area and Xinjiang Toksun County town 3, included the basic data of -12 months of September 2015 a total of 620 patients over the age of 45 Uighur men include: height, weight, waist circumference, blood pressure, laboratory examination results including: sex hormone binding globulin (SHBG), testosterone (TT), fasting blood glucose (FBG), OGTT2h blood glucose, glycosylated hemoglobin (PBG) (HbAlC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), triglyceride (TG), cholesterol (TC), blood uric acid (UA), fasting insulin (Ins), vitamin D (VitD) Osteocalcin (OST), and body mass index calculated by the formula (BMI), insulin resistance index (HOMA-IR). According to the serum SHBG levels were divided into low level and normal level is divided into two groups, the comparison between the two groups of the above indexes, and to explore the relationship between SHBG and the above indexes. Results: the globulin group TT. The low level of elderly male sex hormone binding in Uygur Ins, HOMA-IR, LDL-C were higher than the level of sex hormone binding globulin in normal group, while age, systolic blood pressure, the level of VitD was lower than SHBG normal group, P were less than 0.05. using Spearman rank correlation correlation analysis in Uygur elderly male age, systolic blood pressure, diastolic blood pressure. BMI, HDL-C, VitD, OST were positively correlated with SHBG (r=0.347, r=0.158, r=0.125, r=0.097, r=0.102, r=0.439, r=0.116); FBG, TG, LDL-C, TT, Ins, HOMA-IR were negatively correlated with SHBG (r= 0.167, r=0.088, r=-0.424, r=-0.168, r=-0.725, r=-0.718, P) were less than 0.05. Regression analysis showed that low level of SHBG protection factors were age, HDL-C, VitD, FBG, LDL-C, HOMA-IR, Ins, TT were risk factors. Conclusion: the Uygur elderly men, age, HDL-C, VitD may be the independent protective factors for the low level of SHBG.
