乳腺癌患者心理韧性保护性因素的作用机制研究

【摘要】：目的探讨乳腺癌患者心理韧性保护性因素的相互作用关系,构建心理韧性保护性因素的结构模型,明确心理韧性保护性因素的作用路径,为临床医护人员采取干预措施,提高乳腺癌患者的心理适应能力,改善其生活质量提供理论依据;方法采用横断面研究,调查了2015年10月至2016年8月于安徽省某三甲医院乳腺外科400例乳腺癌患者,采用一般资料问卷、心理韧性量表、一般自我效能量表、生活定向量表修订版、自我掌控感量表、多元领悟社会支持量表进行调查。数据使用Epi Data3.1软件进行录入,SPSS13.0统计软件进行人口学资料、各量表得分和相关系分析,使用Amos17.0软件进行验证性因子分析、结构模型的构建以及中介效应分析。结果(1)乳腺癌患者心理韧性、一般自我效能感、自我掌控感、乐观倾向、领悟社会支持量表总得分分别为78.81±11.53、26.65±5.08、27.91±3.96、18.01±3.43,72.51±8.12,项目均分分别为5.63±0.83、2.66±0.51、3.99±0.57、3.00±0.57、6.04±0.68;(2)相关性分析:心理韧性总分与一般自我效能感、乐观倾向、自我掌控感、社会支持及三个维度的相关性系数在0.198~0.893(p0.05)之间;(3)验证性因子分析发现本研究中使用的量表各项指标适配指标均较好(GFI,AGFI,IFI值多数大于0.90,RMSEA值均小于0.08);(4)分别以领悟社会支持、一般自我效能感、自我掌控感、乐观倾向为中介变量构建结构模型,发现四个结构模型的适配度均达到标准。结论本研究构建的乳腺癌患者心理韧性保护性因素相互作用模型,均具有较好的适配指标。一般自我效能感、乐观倾向、自我掌控感、领悟社会支持对心理韧性产生直接的保护性作用,同时可以通过其中任一个心理韧性保护性因素作为中介变量对心理韧性产生间接的预测效应。
[Abstract]:Objective to explore the interaction of protective factors of psychological toughness in patients with breast cancer, to construct the structural model of protective factors of psychological toughness, to clarify the action path of protective factors of psychological toughness, and to take intervention measures for clinical medical staff. Methods A cross-sectional study was conducted to investigate 400 breast cancer patients from October 2015 to August 2016 in a breast surgery department in Anhui Province. General information questionnaire, mental resilience scale, general self-efficacy scale, life orientation scale, self-control scale, The data were recorded by Epi Data3.1 software and SPSS 13.0 statistical software for demographic data. The scores of each scale and their relationships were analyzed, and the confirmatory factor analysis was performed by Amos17.0 software. Results 1) Psychological resilience, general self-efficacy, self-control, optimistic tendency of breast cancer patients. General self-efficacy, optimism, self-control, social support have direct protective effect on psychological resilience. At the same time, one of the protective factors of psychological toughness can be used as an intermediate variable to indirectly predict the psychological toughness.
