血清betatrophin水平与多囊卵巢综合征的相关性研究

【摘要】:目的:通过测定多囊卵巢综合征患者血清betatrophin水平及对betatrophin与生殖内分泌激素及糖脂代谢紊乱的关系研究,明确betatrophin与多囊卵巢综合征之间的关系,探讨伴有糖脂代谢紊乱的多囊卵巢综合征的发病机制,为多囊卵巢综合征患者的诊断提供依据。方法:1选2016年3月-2016年12月于河北医科大学第一医院生殖医学科门诊就诊的20-40岁的患者,入组患者3个月内未使用过影响体内激素和糖脂水平的药物,无其他内分泌代谢疾病。实验组患者符合2003年鹿特丹会议关于女性PCOS的诊断要求,对照组为非PCOS患者,各20例。对照组患者记录患者年龄,测量患者的身高、体重、腰围、臀围,测定激素六项、AMH,由同一医生行阴道超声检查;观察组除测量对照组的指标外,需要加测雄烯二酮、脱氢表雄酮、性激素结合蛋白,75g葡萄糖耐量试验及胰岛素释放试验、生化全项等,并留取外周静脉血血清,放置-20℃冰箱保存,用ELISA方法,测定血清betatrophin水平。2将患者的临床资料及各指标测量值记录于excel表格中,用spss21.0对数据进行统计分析。结果:1与对照组相比,PCOS患者身高、年龄及各体重指标均无明显统计学差异(P0.05)。2在生殖内分泌指标方面,PCOS患者血清抗缪勒氏管激素、促黄体生成素、总睾酮水平显著升高(P0.01),雄烯二酮明显增高,性激素结合蛋白处于正常范围低值。PCOS组患者表现出更为严重的糖脂代谢紊乱。3 PCOS组血清betatrophin水平较对照组显著升高(P0.05);超重或肥胖组PCOS患者血清betatrophin水平高于体重正常组(P0.05),脂肪中心性分布组PCOS患者血清betatrophin水平高于非中心性分布组(P0.05),高雄激素组PCOS患者血清betatrophin水平高于非高雄激素组(P0.05),胰岛素抵抗组PCOS患者血清betatrophin水平高于非胰岛素抵抗组(P0.05),脂代谢异常组PCOS患者血清betatrophin水平高于非脂代谢异常组(P0.05)。4 Betatrophin与PCOS患者体重指数呈正相关(P0.05);与促黄体生成素及促黄体生成素/促卵泡激素比值、雄烯二酮、游离睾酮指数呈正相关(P0.05),且与性激素结合蛋白呈负相关(P0.05);在糖脂代谢方面,与甘油三酯、空腹胰岛素水平、HOMA-IR、2h血糖呈正相关(P0.05)。结论:1多囊卵巢综合征患者血清betatrophin水平明显增高。2不同类型多囊卵巢综合征患者血清betatrophin水平不同。随着患者体重增加,血清betatrophin水平不断升高;伴有高雄激素血症的多囊卵巢综合征患者血清betatrophin水平明显增高;伴有糖脂代谢紊乱的多囊卵巢综合征患者血清betatrophin水平明显增高。3 Betatrophin有望成为一个多囊卵巢综合征的监测指标。
[Abstract]:Objective: to study the relationship between betatrophin and reproductive endocrine hormone and glycolipid metabolism disorder in patients with polycystic ovary syndrome (PCOS) by measuring the level of serum betatrophin, and to clarify the relationship between betatrophin and polycystic ovary syndrome (PCOS).To explore the pathogenesis of polycystic ovary syndrome (PCOS) with glucose and lipid metabolism disorder, and to provide evidence for diagnosis of PCOS.Methods from March 2016 to December 2016, patients aged 20 to 40 years old from the Department of Reproductive Medicine of the first Hospital of Hebei Medical University were selected.
