累及上颈椎类风湿关节炎患者单纯手术治疗与结合药物治疗的临床对比研究

【摘要】：目的对比研究分风湿关节炎累及上颈椎手术治疗结合药物治疗与学纯后路手术治疗临床疗效。方法回归性分析我院2006-01~2016-01年间门诊及临床收治的典型60例分风湿关节累及上颈椎的患者。将患者分为学纯后路手术治疗组(24例)(A组)与手术治疗结合药物治疗组(36例)(B组),比较两组间患者术前及术后神经症状评估(JOA评分)及疼痛评估(VAS评分)、术前及术后PADI、AADI、SAC及Ranawat值等。结果经过系统性总结分析,手术治疗结合药物治疗组术后JOA及VAS评分较学纯后路手术组明显改善,二组差异有统计申意义(P0.05)。由于分风湿关节炎是卢种缓慢进展性疾病,两组患者术后PADI、AADI、SAC及Ranawat值均较前改善,但随访统计检测数据有限,二者间短时间内差异并不明显,无明显统计申意义。结提对于分风湿关节炎累及上颈椎早提行手术治疗能获得良好的临床症状改善,而手术治疗结合药物控制具有更明显的临床效果。
[Abstract]:Objective to study the clinical effect of treating rheumatoid arthritis with upper cervical vertebra operation combined with drug therapy and pure posterior operation. Methods A regression analysis of 60 patients with rheumatic joint involvement of the upper cervical vertebrae in our hospital from 2006-01 to 2016-01 was carried out. The patients were divided into two groups: group A (n = 24) and group B (n = 36). The preoperative and postoperative neurologic symptom assessment (JOA) and pain assessment (VAS) were compared between the two groups. After operation, PADI AADI, SAC and Ranawat, etc. Results the scores of JOA and VAS in the surgical treatment combined with drug therapy group were significantly improved compared with those in the control group. The difference between the two groups was statistically significant (P 0.05). As rheumatoid arthritis is a slow progressive disease of Lu, the values of PADIAADISAC and Ranawat were improved in both groups after operation, but the statistical data of follow-up were limited, the difference between them was not obvious in a short period of time, and there was no significant statistical significance. Conclusion levitation can improve the clinical symptoms of patients with rheumatoid arthritis involving the upper cervical vertebrae early, but the surgical treatment combined with drug control has a more obvious clinical effect.
