一种面向患者依从度增强的移动医疗软件设计方法研究与实践

【摘要】:如何提高患者依从度是慢病管理面临的主要问题。应用移动医疗技术能提高患者依从度,在慢病管理中越来越受到重视。但在实际应用中,移动医疗软件往往由于其功能设计未能满足用户需求,使得患者依从度的增强达不到预期目标。目标导向设计方法是一种以有效识别用户需求为核心的软件设计方法,在移动医疗软件设计中引入该方法有望提高功能设计的有效性,从而提高患者依从度。因此,本论文对目标导向设计方法在移动医疗领域的应用进行探索,提出一种面向患者依从度增强的移动医疗软件设计方法,以高血压慢病管理软件的设计为例,论述该方法的实践过程,通过试用评估证明该方法的有效性。本论文具体内容包括:(1)移动医疗软件设计方法研究。通过对现有设计方法的比较和剖析,发现目标导向设计方法具有潜在优势,用户模型的建立能够有效识别不同用户需求,从而提高患者依从度。然而已有的用户模型过于抽象化,不足以支持移动医疗软件的设计,因此本论文提出了面向患者依从度增强的目标导向设计方法,重点对用户建模内容进行完善,加入了影响患者依从度的特征因素,并明确了建立用户模型的具体流程。(2)高血压慢病管理软件用户模型建立及需求分析。根据定量和定性研究结果将高血压患者分为三组,形成能够准确描述患者特征及使用目标的用户模型。该模型明确了高血压患者在疾病认知程度、自我管理经验、智能手机使用经验等方面存在的差异,总结了不同患者对慢病管理软件的功能性期望和可用性期望。进一步总结提炼,分析出三组用户的核心需求分别为提高自我管理能力,增强自我管理动力,获取自我管理支持。将用户需求与具体功能对应,形成了高血压慢病管理软件的五个功能模块。(3)高血压慢病管理软件框架定义及原型设计。根据用户建模结果确定使用不同功能模块的主要用户,结合患者特征进行针对性设计:针对缺乏智能手机使用经验的第一类用户,通过可用性测试提高管理计划功能模块的可用性;针对高血压重视程度较低的第二类用户,应用计算机劝导技术加强健康分析报告内容的劝说效果;针对希望得到及时干预的第三类用户,从高血压临床指南中提取相关知识完成血压异常预警服务的设计。(4)高血压慢病管理软件使用效果评估。根据用户建模结果对143名患者进行分组,对不同版本的高血压慢病管理软件进行试用。试用结果表明患者依从度随软件功能的增加逐渐提高,最终维持在较高水平(0.73);访谈结果证明软件的功能设计能够满足不同分组用户的使用需求,是患者依从度提高的主要原因。本论文提出了面向患者依从度增强的目标导向设计方法,通过对高血压慢病管理软件的设计实践,证明该方法能够有效指导移动医疗软件的设计,提高患者依从度。本论文研究结果及实践过程能够为其他移动医疗软件的设计提供参考和借鉴。
[Abstract]:How to improve patient compliance is the main problem in the management of chronic disease. The application of mobile medical technology can improve patient compliance and more and more attention in the management of slow disease. However, in practical application, mobile medical software often fails to meet the needs of the user because of its functional design, so that the enhancement of patient compliance can not reach the expected goal. The objective oriented design method is a software design method based on the effective identification of user requirements. The introduction of this method in mobile medical software design is expected to improve the effectiveness of functional design and improve patient compliance. Therefore, this paper explores the application of the objective oriented design method in mobile medical field, and proposes a kind of application of the target oriented design method in the mobile medical field. The design method of mobile medical software for patient compliance enhanced, taking the design of hypertension slow disease management software as an example, the practical process of this method was discussed. The effectiveness of the method was proved through trial evaluation. The specific contents of this paper include: (1) research on the design method of mobile medical software. By comparing and analyzing the existing design methods, It is found that the target oriented design method has the potential advantage. The establishment of user model can effectively identify different user needs and improve patient compliance. However, the existing user model is too abstract to support the design of mobile medical software. Therefore, the objective oriented design method for patient compliance enhancement is proposed in this paper. To improve the content of user modeling, add the characteristic factors that affect patient compliance, and define the specific process of establishing the user model. (2) the establishment and requirement analysis of the user model of the management software for the hypertension slow disease. According to the quantitative and qualitative results, the hypertensive patients are divided into three groups, forming the accurate description of the patient's characteristics. The model identifies the differences in the degree of cognition, self management experience, and the experience of smart phone use in patients with hypertension, and summarizes the functional expectations and availability expectations of different patients for the management software of the chronic disease. Further summarize and analyze the core requirements of the three groups of users. In order to improve self management ability, enhance self management motivation and obtain self-management support, five functional modules of hypertension slow disease management software are formed by corresponding user needs and specific functions. (3) definition and prototype design of hypertension slow disease management software framework. The main use of different functional modules is determined according to the user modeling results. Targeted design in combination with patient characteristics: to improve the availability of management plan functional modules through usability testing for the first type of users who lack the experience of smart phones; apply the persuasion effect of computer persuasion technology to strengthen the content of health analysis report by using the second types of users with low emphasis on high blood pressure; Third types of users who were expected to intervene in time were expected to design the abnormity warning service of blood pressure from the clinical guidelines of hypertension. (4) the effectiveness evaluation of the management software for the hypertension of the chronic hypertension. 143 patients were grouped according to the results of the user modeling, and the different versions of the high blood pressure and slow disease management software were tried out. The compliance of the patient with the software function increased gradually and finally maintained at a higher level (0.73). The interview results showed that the function design of the software could meet the needs of different groups of users, and the main reason for the improvement of patient compliance. The design practice of the management software of blood pressure slow disease proves that the method can effectively guide the design of mobile medical software and improve patient compliance. The results and practice of this paper can provide reference and reference for the design of other mobile medical software.
