呼吸系统粘液表皮样癌一例报告并文献复习

【摘要】：目的探讨呼吸系统粘液表皮样癌的临床特征及诊治现状。方法对2016年在浙江大学医学院附属邵逸夫医院收治的1例呼吸系统粘液表皮样癌患者并复习国内外相关文献,通过收集的共7个病例对该疾病进行回顾性分析。结果本组患者的中位年龄40岁,年龄跨度为6-71岁,除2个患者为6岁儿童,1个患者为22岁青年外,其他均为成人且年龄大于40岁(57%)。7个病例中,男5例,女2例。本组5个病例有精确的肿块大小记载,平均最大直径为3.0cm。肿块位于右肺的有5例,左肺的有1例,累及左肺上下两叶,气管内1例。本组Ⅰ期患者4例,其中2例因淋巴结状态未知而不能肯定。Ⅱ期患者1例。111期患者1例为T4N2M0。本组6例可手术患者均行手术治疗,均为根治性手术,术后所有患者均未行辅助治疗,仅有1例晚期患者无法手术,予姑息放化疗治疗。本组患者无病生存期均超过随访的6-12个月。本文报道的患者自行气管肿瘤切除术以来存活已超过12个月。所有7位患者都无吸烟史,可以大致推断吸烟不是呼吸系统粘液表皮样癌的危险因素。结论呼吸系统粘液表皮样癌大气道的侵犯非常典型,多数患者可表现为咳嗽、咯血、呼吸困难、发热。术前诊断困难,容易误诊,特别是仅仅通过穿刺的诊断与肺腺癌很难区分。其确诊需手术后病理结果证实。手术是治疗本病最主要的方法,晚期的辅助治疗可联系放疗及化疗。可手术患者根治术后预后显著好于其他不可手术切除的患者。
[Abstract]:Objective to investigate the clinical features, diagnosis and treatment of mucoepidermoid carcinoma of respiratory system. Methods A case of mucoepidermoid carcinoma of the respiratory system was treated in the affiliated run Shaw Hospital of Zhejiang University Medical College in 2016 and the related literatures were reviewed and analyzed retrospectively through the collection of 7 cases. Results the median age of the patients was 40 years, the age span ranged from 6 to 71 years old. Except 2 patients were 6 years old children and 1 patient was 22 years old young people, all the patients were adults and over 40 years old (57%). Among the 7 cases, 5 cases were males.
