尤瑞克林对脑梗死患者神经功能及血清基质金属蛋白酶9的影响The effect of Kallikrein on neurological function and serum matrix metalloproteinase-9 in patients with cerebral infarction


【摘要】目的 探讨尤瑞克林对脑梗死患者神经功能及MMP-9的影响。方法 选择我院神经内科诊治的急性脑梗死患者90例,随机分为观察组和对照组,在常规治疗的基础上,对照组患者应用疏血通,观察组患者尤瑞克林,治疗周期2周。采用NIHSS评分及MRS评分评价患者神经功能缺损程度,应用ELISA法检测血清MMP-9水平。结果 1.观察组患者治疗效果优于对照组(P<0.05)。2.两组患者治疗3月后NIHSS评分及MRS评分明显降低(P<0.05)。治疗3月后,观察组患者NIHSS评分及MRS评分降低程度均较对照组显著(P<0.05)。3.治疗2周后两组患者血清MMP-9水平、血粘度及红细胞压积均有改善,且观察组患者较对照组改善明显(P<0.05)。结论 尤瑞克林可以显著改善脑梗死患者神经功能,明显降低其血清MMP-9水平,,具有很高的临床应用价值。

[Abstract] Objective To investigate the effect of Kallikrein on neurological function and serum MMP-9 in patients with cerebral infarction. Methods 90 patients with acute cerebral infarction who were treated in our hospital were chosen, They were randomly divided into two groups, On the basis of conventional therapy,The control group were treated with Shuxuetong,the observation group were treated with Kallikrein, for 2 weeks. NIHSS score and MRS score were used to evaluate the neurologic impairment, serum MMP-9 levels were tested using ELISA assay. Results 1. the effect of the the observation group were better than the control (P <0.05). The NIHSS score and the MRS score were significantly reduced in both groups after 3 months treatment(P <0.05). 3 months after treatment, the decrease of NIHSS score and the MRS score of the observation group were significantly higher than the control (P <0.05).  3. 2 weeks after treatment, the serum MMP-9 levels、 blood viscosity and hematocrit were improved, and the improvement in the observation group were significantly than that of the control(P<0.05). Conclusion Kallikrein can significantly improve neurological function in patients with cerebral infarction, significantly decrease serum MMP-9 levels, it high clinical application value.

脑梗死是神经内科常见病,它发病急骤,部分病情危重,有很高的致残和致死率,严重影响了患者的生命安全和生活质量。及时、准确的诊断和合理的治疗对其预后十分重要。血清基质金属蛋白酶9(matrix metalloproteinase-9,MMP-9)在脑缺血后脑组织损伤中起重要作用。尤瑞克林可以通过对低分子量激肽原的作用等途径改善脑组织的血液灌注,促进缺血脑组织功能的恢复[1],但目前关于其对患者MMP-9水平影响情况的报道较少。本研究中,笔者通过回顾性分析90例急性脑梗死患者的临床资料,探讨尤瑞克林对脑梗死患者神经功能及MMP-9的影响。


1 资料与方法


1.1一般资料 回顾性分析2009年10月至2010年10月间我院神经内科诊治的90例急性脑梗死患者的临床资料。纳入条件:年龄50-70岁;患者均为首发急性脑梗塞,并经颅脑MRI的DWI检查,发现明确脑梗塞病灶;发病至用药时间≤12小时;此前未长期口服阿司匹林,氯吡格雷等抗血小板聚集药物;脑梗塞病变血管为大脑中动脉及大脑后动脉。排除条件:大面积脑梗塞;房颤、心梗等心脏疾病;血液系统疾患;严重肝功能、肾功能不全;精神病病史及家族史;尤瑞克林过敏史。应用MR后处理工作站中的测量工具测量梗死体积,测量范围不含周围水肿区。

综上所述,尤瑞克林可以显著改善脑梗死患者神经功能,明显降低其血清MMP-9水平,有较好的疗效,具有很高的临床应用价值。
