合作原则指导下的医患门诊会话研究

【摘要】:合作原则是语用学的一条重要原则。同时,它也是指导人们会话和交际的重要原则,因此它也引起越来越多的研究者的关注。目前,在国内和国外,对合作原则的一般性研究比较多,但是对于机构话语的研究却很少。作为一种重要的机构语言,医患会话与我们的日常生活紧密联系。因此,本文主要围绕医患会话中的合作问题来进行研究。本文研究对象为中国医院门诊室内医生和患者在会诊时的对话。本研究语料是作者于2014年1-2月在山西省某所三甲医院的7大门诊内收集的。本文采用的12例医患对话是从49例录音语料中随机抽取。本研究在全面综述了国内外医患互动相关文献的基础上,通过转写和翻译从山西省某所三甲医院门诊室内收集的录音语料,创建了合作原则指导下的医患会话分析模式的理论框架。该理论框架,从语用学的角度分析了医患门诊会话中医患双方对新合作原则的遵循和违背情况及其所反映的当今中国的医患关系。本文主要根据新合作原则的五个准则,分析医生和患者在医患门诊会话的各个阶段中对五个准则的遵守和违背情况。研究结果表明:(1)在回答医生的提问时大部分患者会违反量准则。(2)许多患者会违反相关准则。(3)许多患者通常会违反方式准则。(4)大部分医生通常遵守质准则而违反量准则。(5)医生和患者都可能违反态度准则,而大部分患者都会违反态度准则。总之,在医患门诊会话的六个阶段中,医生和患者都应遵守新合作原则的五个准则,这样才能促进成功的医疗和友好的医患关系。
[Abstract]:The principle of cooperation is an important principle of pragmatics. At the same time, it is also an important principle to guide people's conversation and communication, so it has attracted more and more researchers' attention. At present, there are many general studies on cooperative principles at home and abroad, but few studies on institutional discourse. As an important institutional language, doctor-patient conversation is closely related to our daily life. Therefore, this paper focuses on the cooperation between doctors and patients. The object of this study is the dialogue between the indoor doctor and the patient in the consultation of Chinese hospital outpatient. The corpus of this study was collected in 7 outpatient clinics of a third Class A Hospital in Shanxi Province in January and February 2014. Twelve cases of doctor-patient dialogue were randomly selected from 49 recorded data. On the basis of a comprehensive review of the literature on doctor-patient interaction at home and abroad, this study passes through the transliteration and translation of the recorded language collected from the outpatient clinic of a third Class A Hospital in Shanxi Province. A theoretical framework of doctor-patient conversation analysis model under the guidance of cooperative principle is established. From the perspective of pragmatics, this paper analyzes the adherence and violation of the new principle of cooperation between doctors and patients in the doctor-patient clinic conversation and the relationship between doctors and patients in China today. According to the five principles of new cooperation, this paper analyzes how doctors and patients abide by and violate the five principles in each stage of doctor-patient clinic conversation. The results show that: 1) when answering a doctor's question, most patients will violate the guideline of quantity. (2) many patients will violate the relevant criterion. 3) many patients will usually violate the standard of way. 4) most doctors usually follow the quality criterion and violate the quantity. Guideline No. 5) both doctors and patients may violate the attitude guidelines, Most patients violate attitude guidelines. In conclusion, in the six stages of doctor-patient conversation, both doctors and patients should abide by the five principles of new cooperation, so as to promote successful medical care and friendly doctor-patient relationship.
