【摘要】:背景:外科手术是治疗脊柱结核的重要手段。但考虑脊柱位置的特殊性,承受机体应力及需拆除内固定所致二次损伤是外科治疗的局限所在。组织工程为各种疾病的治疗带来了新的思路和希望。理想生物材料代替传统内固定材料,具有明显无毒害及明显免疫排斥反应。目的:探究海藻酸盐结合骨髓间充质干细胞修复脊柱结核的力学性能。方法:将40只新西兰兔等分为对照组、脊柱结核组、海藻酸盐组与钛合金组。对照组不予以任何处理,其他组于L_5近椎间盘处钻孔填充明胶海绵,注射结核菌混悬液0.1 mL(菌量5 g/L),建立脊柱结核模型。2个月后,后2组模拟L_(4-5)脊柱结核切除,植入复合骨髓间充质干细胞的藻酸盐凝胶材料和钛合金内固定材料。结果与结论:钛合金组1只新西兰兔出现局部感染、红肿;海藻酸盐组未出现局部感染、红肿等不良反应。海藻酸盐组及钛合金组前屈、后伸及侧弯距离及最大轴向拔出力均优于脊柱结核组,均接近于对照组。结果说明复合骨髓间充质干细胞的藻酸盐凝胶材料植入后与机体生物相容性较好,且复合骨髓间充质干细胞的藻酸盐凝胶材料植入后脊柱稳定性与钛合金接近,具有良好的脊柱稳定性。
[Abstract]:Background: surgery is an important method for the treatment of spinal tuberculosis. The limitation of surgical treatment lies in the secondary injury caused by the stress of the body and the need to remove the internal fixation. Tissue engineering brings new ideas and hopes for the treatment of various diseases. Ideal biomaterials replace the traditional internal fixation materials. Objective: to investigate the mechanical properties of alginate combined with bone marrow mesenchymal stem cells (BMSCs) in the repair of spinal tuberculosis. Methods: 40 New Zealand rabbits were divided into control group. Spinal tuberculosis group, alginate group and titanium alloy group. The control group was not treated with any treatment. The other groups were drilled and filled with gelatin sponge at L5 near the intervertebral disc. The spinal tuberculosis model was established by injecting tuberculous bacillus suspension 0.1 mL (5 g / L). Two months later, the latter two groups were treated with simulated LSP 4-5 spinal tuberculosis resection. Alginate gel material and titanium alloy internal fixation material were implanted with bone marrow mesenchymal stem cells. Results and conclusion: one New Zealand rabbit in titanium alloy group had local infection and swelling. Alginate group did not show local infection, swelling and other adverse reactions. Alginate group and titanium alloy group forward flexion, extension and lateral bending distance and maximum axial pull-out force were better than spinal tuberculosis group. The results showed that the alginate gel material combined with bone marrow mesenchymal stem cells had good biocompatibility after implantation. The spinal stability of the composite bone marrow mesenchymal stem cells implanted with alginate gel was similar to that of titanium alloy.
