MecA基因在金黄色葡萄球菌中的分布和对耐药性的影响

【摘要】:目的 探讨mecA基因在金黄色葡萄球菌(Staphylococcus aureus,S.aureus)中的分布以及mecA基因对金黄色葡萄球菌耐药性的影响。 方法 收集金黄色葡萄球菌临床分离菌株47例,选取12种针对葡萄球菌最常用的抗菌药物(苯唑青霉素、青霉素、氨苄西林、哌拉西林、阿莫西林棒酸、头孢噻吩、头孢噻肟、头孢吡肟、左旋氧氟沙星、丁胺卡那霉素、替考拉宁以及万古霉素),用琼脂扩散法(改良Kirby-Bauer法)进行敏感性检测。提取并纯化47例临床金黄色葡萄球菌分离株DNA,通过聚合酶链反应(PCR)对mecA基因进行扩增,分析mecA基因和金黄色葡萄球菌耐药性的关系,同时结合病人临床资料进行综合分析。 结果 ①47株金黄色葡萄球菌中,有33株为耐甲氧西林的金黄色葡萄球菌(MRSA),占70.2%;甲氧西林敏感型金黄色葡萄球菌(MSSA)有14株,占29.8%。在33株MRSA中,只有3株对除替考拉宁、万古霉素等糖肽类以外的抗菌药物敏感,占9.1%;14株MSSA中只有2株对全部12种抗菌药物敏感,仅占14.3%。②PCR结果显示,在33株MRSA中,32株携带mecA基因,占97.0%,只有1株不携带mecA基因。而在14株MSSA中有3株携带mecA基因,3株携带mecA基因的MSSA耐药性较不携带mecA基因MSSA的耐药性严重。③47例金黄色葡萄球菌感染中45例属于医院感染,其中15例属于暴发感染。 结论 ①金黄色葡萄球菌中耐甲氧西林菌株(MRSA)具有较高的分离率。②MRSA的耐药性非常严重,万古霉素等糖肽类抗生素是唯一有效的抗菌药物。MSSA的耐药性也较严重。③绝大多数的MRSA携带有mecA基因,mecA基因在金黄色葡萄球菌的耐药机制中发挥重要作用。有少数MSSA携带有mecA基因。携带mecA基因的MSSA耐药情况比不携带mecA基因的MSSA严重。④本地区金黄色葡萄球菌的感染仍以医院感染为主;在医院内容易发生金黄色葡萄球菌感染的暴发。
[Abstract]:Objective to investigate the distribution of mecA gene in Staphylococcus aureus (S.aureus) and the effect of mecA gene on the resistance of Staphylococcus aureus.
Methods the clinical isolates of Staphylococcus aureus in 47 cases, selected 12 kinds of antibacterial drugs for Staphylococcus aureus is the most commonly used (oxacillin, penicillin, ampicillin, piperacillin, amoxicillin clavulanic acid, cephalothin, cefotaxime, cefepime, levofloxacin, amikacin and teicoplanin. Vancomycin), using agar diffusion method (modified Kirby-Bauer method) were extracted and purified. The sensitivity of detection of 47 clinical isolates of Staphylococcus aureus DNA by polymerase chain reaction (PCR) of mecA gene were amplified by analysis of the relationship between mecA gene and drug resistance of Staphylococcus aureus, and the clinical data of patients were analyzed.
Results 47 strains of Staphylococcus aureus, Staphylococcus aureus 33 strains of methicillin resistant (MRSA), accounting for 70.2%; methicillin sensitive Staphylococcus aureus (MSSA) accounted for 29.8%. in the 14 strains, 33 strains of MRSA, only 3 strains of teicoplanin, vancomycin outside sugar peptide antimicrobial susceptibility of 14 strains of MSSA, accounting for 9.1%; only 2 of all strains were sensitive to 12 kinds of antibiotics, accounting for only 14.3%. of PCR showed that the 33 strains MRSA, 32 strains carried mecA gene, accounted for 97%, only 1 strains carrying mecA gene in 14 strains of MSSA, 3 strains carrying mecA gene, 3 strains of resistance of MSSA carrying mecA gene carrying mecA gene MSSA is not serious. In 47 cases of Staphylococcus aureus infection in 45 cases of hospital infection, including 15 cases belonging to the outbreak of infection.
Methicillin resistant strains of Staphylococcus aureus (MRSA) separation rate is very high. The resistance of MRSA is very serious, glycopeptides antibiotics are only effective antimicrobial drug resistance of.MSSA is more serious. The vast majority of MRSA carrying mecA gene, mecA gene play an important role in drug resistance the mechanism of Staphylococcus aureus. There are a few MSSA carrying mecA gene. Carrying MSSA resistance gene mecA is not carrying mecA gene MSSA. The serious region of Staphylococcus aureus infection in hospital infection; susceptible to hospital outbreaks of Staphylococcus aureus infection.
