低位腹主动脉阻断术的解剖观察

【摘要】： 目的对腹主动脉下段的解剖关系进行研究,找出腹主动脉下段阻断的最佳途径及部位,为低位腹主动阻断术的顺利完成提供解剖方面的资料。方法对27具尸体标本进行解剖,找出腹主动脉下段的主要分支,测量分支间的距离及与腹主动脉下段相邻的重要组织器官的距离,进行统计学处理。结果肠系膜下动脉到主动脉分叉最低点的距离为4.27±0.8cm。肠系膜下动脉到第三腰横动脉的距离为0.46±0.52cm。第三、四腰横动脉的距离为2.18±0.48cm。第四腰横动脉到骶正中动脉的距离为1.09±0.62cm。骶正中动脉到腹主动脉分叉最低点的距离为0.53±0.37cm。第三、四腰横动脉水平腹主动脉壁至相应侧输尿管的距离为:第三,左2.6±0.37cm,右3.56±0.3;第四,左2.4±0.38cm,右3.34±0.35cm。腹主动脉与下腔静脉相邻壁间距离为:第三腰横动脉水平0.73±0.34cm,第四横动脉水平0.53±0.32cm。结论第三、四腰横动脉间的距离最宽,适合在此进行低位腹主动脉阻断术,第三、四腰横动脉间的中点至主动脉分叉最低点距离为:2.7±0.62cm,阻断点宜在此处;肠系膜下动脉至主动脉分叉最低点的距离为:4.27±0.8cm,不应超过此距离;腹主动脉与下腔静脉很近,分离时应注意避免损伤静脉壁。
[Abstract]:Objective to study the anatomical relationship of the lower segment of the abdominal aorta and to find out the best way and location of the inferior segment of the abdominal aorta. Methods 27 cadavers were dissected and the main branches of lower abdominal aorta were found. The distance between branches and the important tissues and organs adjacent to the lower segment of abdominal aorta were measured. Results the distance from the inferior mesenteric artery to the lowest point of aortic bifurcation was 4.27 卤0.8 cm. The distance from the inferior mesenteric artery to the third transverse lumbar artery was 0.46 卤0.52 cm. Three. The distance between the four transverse artery and the median sacral artery was 2.18 卤0.48 cm 路4th cm. The distance from the median sacral artery to the lowest point of the abdominal aortic bifurcation was 0. 53 卤0.37 cm. Third. The distance between the horizontal abdominal aorta wall of the four lumbar transverse arteries and the corresponding lateral ureter was as follows: third, left 2.6 卤0.37 cm, right 3.56 卤0.3; 4th, left 2.4 卤0.38 cm, right 3.34 卤0.35 cm. The distance between abdominal aorta and inferior vena cava was 0.73 卤0.34 cm. 4th horizontal artery level was 0.53 卤0.32 cm. Conclusion the distance between the third and fourth lumbar transverse arteries is the widest, which is suitable for low abdominal aortic occlusion. The distance between the midpoint of the four lumbar transverse arteries and the lowest point of aortic bifurcation is 7. 7 卤0. 62 cm, where the blocking point should be. The distance from the inferior mesenteric artery to the lowest point of aorta bifurcation was 4. 27 卤0. 8 cm, which should not be exceeded. The abdominal aorta is close to the inferior vena cava.
