大鼠肢体缺血—再灌注后海马Fas蛋白表达的变化

【摘要】:肢体缺血-再灌注(I-R)是肢体挤压伤、断肢再植术及腹主动脉瘤手术等患者走向康复的病理转归过程,肢体严重缺血后再灌注可引发多器官功能衰竭(MOF),MOF 是此类患者最严重的并发症,其死亡率占重症监护患者死亡率的30-40%。因此,探讨肢体I-R 引发多器官损伤的机制,一直是创伤外科及重症医学共同关注的重大课题。 既往的研究多集中在肾、肺等脏器上,本课题组的研究则发现,大鼠肢体I-R 后其海马区锥体层神经元也出现明显的病理改变。海马是与学习、记忆及免疫调节密切相关的中枢部位,其病损可能是外周创伤导致记忆缺失及免疫功能低下的重要原因。因此,阐明肢体I-R 引发海马神经元损伤的机制,具有重要的理论意义和临床应用价值。 Fas 蛋白是肿瘤坏死因子受体家族成员,受其配体Fasl激活后可启动细胞的凋亡程序。在正常脑的海马区,Fas 蛋白仅维持极低的表达水平,但脑I-R 后海马区神经元Fas 蛋白的表达水平明显上调。肢体I-R 所致脑损伤与脑自身I-R损伤的机制不同,Fas 蛋白是否参与介导了肢体I-R 后的海马损伤,尚无研究证实。本实验拟应用免疫组化染色方法,观测大鼠肢体I-R 后海马区Fas 蛋白表达的时空变化规律,为最终阐明肢体I-R 引发脑损伤的机制积累资料。 目的:通过观测大鼠肢体缺血再灌注后海马神经元Fas蛋白表达的变化,证明Fas 蛋白是否介入了肢体I-R 所致脑
[Abstract]:Limb ischemia-reperfusion (I-R) is the pathological process of limb compression injury, amputated limb replantation and abdominal aortic aneurysm surgery. MOF / MOF is the most serious complication in patients with severe limb ischemia and reperfusion. The mortality rate is 30-400.Therefore, the mechanism of limb I-R induced multiple organ injury is discussed. Trauma surgery and critical medicine have been a major issue of common concern. Previous studies have focused on the kidneys, lungs and other organs. Our team found that the hippocampal pyramidal neurons also showed pathological changes after I-R. The lesion of central part closely related to memory and immune regulation may be an important cause of memory loss and low immune function caused by peripheral trauma. Therefore, the mechanism of hippocampal neuron injury induced by limb I-R is clarified. It has important theoretical significance and clinical application value. Fas protein is a member of the tumor necrosis factor receptor family and activated by its ligand Fasl can initiate the apoptosis process. However, the expression of Fas protein in hippocampal neurons after I-R was significantly up-regulated. The mechanism of brain injury induced by limb I-R is different from that of brain I-R. Whether FAS protein is involved in the hippocampal injury induced by I-R in limbs. The present study intends to observe the temporal and spatial changes of Fas protein expression in hippocampal region of rats after limb I-R by immunohistochemical staining, in order to clarify the mechanism of brain injury induced by limb I-R. Objective: to observe the changes of Fas protein expression in hippocampal neurons after limb ischemia-reperfusion in rats, and to prove whether Fas protein is involved in the brain induced by limb I-R.
