5-氮胞苷体外诱导人骨髓间充质干细胞向心肌样细胞分化的实验研究

【摘要】:近年来,急、慢性缺血性心脏病(ischemic heart disease,IHD)的基础研究和临床治疗均有较大的进展,尤其是心血管药物治疗学和心脏介入技术水平不断提高,急性心肌梗死(acute myocardial infarction,AMI)的死亡率明显下降,但心肌梗死后心功能衰竭的发病率和致残率仍居高不下,已成为危害人类健康的主要疾患之一。目前临床治疗缺血性心脏病的常用治疗手段主要包括药物治疗、内科介入治疗、外科手术治疗等。这些治疗方法的主要作用是改善局部血运,挽救濒死心肌,增加存活心肌功能,而非针对病变冠状动脉的再造和梗死心肌再生、重建的根本性治疗方法。干细胞的研究和应用为心肌坏死性疾病提供了新的治疗方法。 细胞治疗(cellular therapy)通过移植功能细胞,替代、修复或加强受损的组织或器官的生物学功能,已成为治疗多种组织坏死性疾病的新策略。细胞移植(cellular transplantation)、心肌再生(myocardial regeneration),可以增加梗死区功能心肌细胞数量,改善心肌功能,逆转心室重塑,是目前研究的热点。骨髓间充质干细胞(mesenchymal stem cell,MSCs)因其独特的生物学特性在干细胞研究领域倍受重视。骨髓中有两种类型干细胞,造血干细胞(hematopoietic stem cell,HSC)和间充质干细胞。骨髓MSCs是存在于骨髓基质内的非造血细胞来源的细胞亚群,是中胚层发育的早期细胞,不仅可以分化为造血实质和造血基质细胞,还可以分化为许多造血以外的组织,特别是中胚层和神经外胚层来源组织的细胞。研究发现MSCs在体内或体外诱导条件下可以分化为心肌样
[Abstract]:In recent years, the basic research and clinical treatment of ischemic heart disease (IHD) in patients with acute and chronic ischemic heart disease have made great progress, especially the level of cardiovascular drug therapy and cardiac interventional techniques. The mortality rate of acute myocardial myocardial infarction (AMI) decreased significantly, but the incidence and disability rate of heart failure remained high after myocardial infarction. It has become one of the major diseases endangering human health. At present, the commonly used methods of clinical treatment for ischemic heart disease include drug therapy, interventional therapy of internal medicine, The main effects of these treatments are to improve local blood flow, save the dying myocardium, and increase the function of the viable myocardium, rather than for the reconstruction of the diseased coronary artery and the regeneration of the infarcted myocardium. The research and application of stem cells provide a new treatment for myocardial necrosis disease. To replace, repair, or strengthen the biological function of a damaged tissue or organ by transplanting functional cells. Cell transplantation and myocardial regeneration can increase the number of myocardial cells in infarcted area, improve myocardial function and reverse ventricular remodeling. Bone marrow mesenchymal stem cells have attracted much attention in the field of stem cell research because of their unique biological characteristics. There are two types of stem cells in bone marrow. Hematopoietic stem cells and mesenchymal stem cells. Bone marrow MSCs is a subgroup of non-hematopoietic cells that exist in bone marrow stroma and is an early stage of mesoderm development. It can not only differentiate into hematopoietic parenchyma and hematopoietic stromal cells, but also differentiate into hematopoietic parenchyma and hematopoietic stromal cells. It can also differentiate into cells from many tissues other than hematopoiesis, especially mesoderm and neuroectoderm. It has been found that MSCs can differentiate into cardiomyocytes in vivo or in vitro.
