肝十二指肠韧带及相关区域的解剖学研究

【摘要】： 肝十二指肠韧带位于肝门与十二指肠上部之间,不论在肝胆外科还是在胃肠外科,均占有极其重要的地位,有非常多的手术均需对此结构进行解剖。其内有进出肝门的肝外胆道、肝固有动脉、门静脉等重要结构,以及肝神经丛、淋巴管和为数不多的淋巴结等。本文利用35具防腐成人腹部标本解剖,观测肝十二指肠韧带内主要结构及其重要分支的结构关系,进行量化分析,同时对各重要结构的起源及与其关系密切的血管也进行了观察,结果发现,其内主要三大结构相对关系比较固定,但其在肝十二指肠内分叉的高低及距离差异较大,以胆道最为明显,它们与各自的上级管通及下级分支之间也存在较多变异。因此详细了解该区的解剖结构,熟悉相关的各种变异,对于此部位的手术具有重要的作用。
[Abstract]:The hepatoduodenal ligament is located between the hepatic hilum and the upper part of the duodenum, and plays an extremely important role both in hepatobiliary surgery and in gastrointestinal surgery. There are a lot of surgeries that need to be dissected. There are important structures in and out of the hilum of the liver, including the extrahepatic bile duct, the proper hepatic artery, the portal vein, and the hepatic nerve plexus. The lymphatic vessels and a few lymph nodes were dissected from 35 antiseptic adult abdominal specimens. The main structures and important branches of the hepatoduodenal ligament were observed and analyzed quantitatively. At the same time, the origin of the important structures and the vessels closely related to them were also observed. The results showed that the relative relations of the three major structures were relatively fixed, but the height and distance of the bifurcation in the liver and duodenum were quite different. The bile duct is the most obvious, and there are many variations between them and their superior canal and their subordinate branches. Therefore, it is important to understand the anatomical structure of the area in detail and to be familiar with the related variations.
