胸腰段脊神经根走行断层解剖、CT和MRI的研究

【摘要】:胸腰脊神经根自硬膜囊离开后,直至从椎间管外口穿出,均经过一条较窄的骨纤维性管道,此通道分为两段,第一段为神经根管,即从硬膜囊穿出点至椎间管内口,第二段为椎间管,即从椎间管内口至椎间管外口。胸腰脊神经根受挤压是引起上下运动神经元症状及感觉异常的常见病因之一,其受挤压部位和原因很多,现对脊神经根走行过程中脊神经节、神经鞘的有关结构进行解剖学和影像学观察和测量,以期为临床诊治胸腰段神经根病提供客观依据。 第一部分 胸腰段脊神经根走行及其相关韧带结构的MRI研究 目的: 通过对四具尸体胸腰段脊柱脊髓的MRI薄层扫描研究,观察测量脊神经根及其根鞘走行过程中长度、横径及倾斜度等内容,并明确各结构在常规和斜位(与神经根走行平行或垂直)MR成像上的正常或异常表现,为研究腰腿疼痛等疾病与胸腰神经根鞘的关系提供解剖学依据。 材料与方法: 选用4具尸体、1例正常男性和1例正常女性活体胸腰节段脊髓及脊神经根分别进行水平横轴位、冠状位、矢状位和与椎间管平行或垂直位TWI扫描。观察脊神经根自硬膜囊腋部发出后至椎间管外口之间一段的解剖关系,测量脊神经根鞘全长,脊神经节内段长度、节段长度、节内段横径、脊神经节近端、远端和中央最膨大处的直径以及根鞘与硬膜囊的夹角(锐角)。 结果: 1.从T_(11)~T_(12)脊神经根鞘的长度和横径稳定在一水平上,变化趋势不大,脊神
[Abstract]:After the thoracolumbar spinal nerve root left from the dural sac, it passed through a narrow fibrous osseous canal from the epidural sac to the intervertebral canal, the first segment being the nerve root canal, that is, from the point of dural sac to the intervertebral canal, the first segment was the nerve root canal, that is, from the puncture point of the dural sac to the intervertebral canal orifice.The second stage is intervertebral canal, that is, from intervertebral canal to intervertebral canal.Compression of thoracolumbar spinal nerve roots is one of the common causes of symptoms and sensory abnormalities of upper and lower motor neurons.The related structures of nerve sheath were observed and measured by anatomy and imaging in order to provide objective basis for clinical diagnosis and treatment of thoracolumbar radiculopathy.Part I: MRI study on the structure of thoracic and lumbar spinal nerve root and its associated ligamentsObjective:The length, transverse diameter and inclination of spinal nerve root and root sheath were measured by MRI thin slice scanning of thoracolumbar spine and spinal cord in four cadavers.The normal or abnormal manifestations of the structures on conventional and oblique (parallel or vertical) Mr imaging with nerve roots were determined, which provided anatomic basis for the study of the relationship between the pain of waist and leg and the sheath of thoracolumbar nerve root.Materials and methods:The thoracolumbar spinal cord and spinal nerve root of 4 cadavers and 1 normal male and 1 female were examined in horizontal axial, coronal, sagittal and parallel or perpendicular to intervertebral canal respectively.The anatomical relationship of spinal nerve root from axillary region of dural sac to the orifice of intervertebral canal was observed. The length of the sheath of spinal nerve root, the length of internal segment of spinal ganglion, the length of segment, the transverse diameter of inner segment and the proximal end of spinal ganglion were measured.The diameters of the most distally and central distally inflated areas and the angle between the root sheath and the dural sac.Results:1.(1) the length and transverse diameter of the root sheath of the spinal nerve are stable at a certain level, and the trend of change is not obvious.
