大鼠原位肝移植及急性排斥反应实验模型建立的初步研究

【摘要】:目的:肝移植是各种终末期肝病的最有效治疗方法,而急性排斥反应又是肝移植后最常见的并发症和死亡原因。大鼠是移植研究中一种较为优越的实验动物,我们参照国内外大鼠肝移植手术方法并加以改进,拟建立一种简易、可靠的动物模型以供临床进一步研究。目前就急性排斥反应的诊断而言,目前尚缺乏特异性及敏感性均令人满意的诊断方法。本研究旨在通过检测肝移植大鼠外周血中细胞因子IFN-γ的含量,以探讨它们对大鼠肝移植后急性排斥反应的诊断和预测价值。 方法:采用“二袖套管法”,通过建立Wistar-Wistar大鼠肝移植模型熟练建立大鼠肝移植模型的技能。继而将实验大鼠共分2组,每组均为15只大鼠,研究同种异体组(SD-Wistar组)、同基因组(Wistar—Wistar组)组的肝移植后各项指标,分析各组大鼠死亡原因,观察术后不同阶段各组症状体征、肝功能变化、移植肝病理特征以及细胞因子IFN-γ等的变化。 结果:经过熟悉后,我们在第二阶段共行大鼠原位肝移植实验37次,手术成功率为91.9%。同种异体组的大鼠平均生存时间为25天,而同基因组大鼠可长期存活,几乎无排斥反应,且术后肝功能恢复迅速。同种异体组肝功能明显受损,且恢复缓慢,病理检查可见明显排斥反应。同种异体组大鼠外周血中IFN-γ的含量明显高于同基因组(P0.01),有明显统计学意义。 结论:成功建立了大鼠原位肝移植的模型并加以改进。肝移植模型的特点:①肝上下腔静脉采取直接缝合;②肝下下腔静脉、门静脉套管吻合,缩短了无肝期时间;③套管自制,节省了费用,并且可以根据不同血管管径选择不同套管。完善的手术技术是建立稳定肝移植模型的关键。SD-wistar肝移植模型鼠有明显急性排斥反应的特征,Wistar-Wistar肝移植模型鼠几无排斥反应发生,
[Abstract]:Objective: liver transplantation is the most effective treatment for various end-stage liver diseases, and acute rejection is the most common complication and cause of death after liver transplantation. Rat is a superior experimental animal in transplantation research. We propose to establish a simple and reliable animal model for further clinical study by referring to and improving the methods of liver transplantation in rats at home and abroad. For the diagnosis of acute rejection, there is no satisfactory diagnostic method of specificity and sensitivity. The purpose of this study was to investigate the diagnostic and predictive value of cytokines IFN- 纬 in peripheral blood of liver transplantation rats. Methods: two sleeve cannula method was used to establish Wistar-Wistar rat liver transplantation model skillfully. Then the experimental rats were divided into two groups, 15 rats in each group. The indexes of liver transplantation in SD-Wistar group and Wistar Wistar group were studied, and the causes of death in each group were analyzed, and the symptoms and signs of each group were observed at different stages after operation. Changes of liver function, pathological features and cytokines IFN- 纬 of transplanted liver. Results: after familiarity, we performed 37 orthotopic liver transplantation experiments in the second stage, and the success rate was 91.9%. The average survival time of allogeneic rats was 25 days, while the same genome rats could survive for a long time, almost no rejection, and the liver function recovered rapidly after operation. The liver function of allograft group was obviously damaged, and the recovery was slow, and the rejection was obvious in pathological examination. The level of IFN- 纬 in peripheral blood of allogeneic group was significantly higher than that of homogenetic group (P 0.01). Conclusion: the rat orthotopic liver transplantation model was successfully established and improved. The characteristics of liver transplantation model were as follows: direct suture of inferior inferior vena cava via direct suture of inferior vena cava, portal venous cannula anastomosis, shortening the time of anhepatic stage and saving cost. Different cannula can be selected according to different vessel diameter. Perfect surgical technique is the key to establish stable liver transplantation model. SD-wistar liver transplantation model mice have obvious acute rejection characteristics. There is almost no rejection in Wistar Wistar liver transplantation model rats.
