【摘要】：前言 2型糖尿病主要的病理生理改变是胰岛素抵抗(Insulin resistance,IR)和胰岛素分泌缺陷,两者相互作用共同导致疾病的发生和发展。目前尚无有效的药物能同时针对2型糖尿病发病机制的两个方面即IR和胰岛素分泌缺陷进行治疗。脂联素(adiponectin)是脂肪细胞分泌的特异性蛋白质,具有增加胰岛素敏感性,减轻IR,降低血糖、血脂、抗炎,抗动脉粥样硬化等作用。adiponectin的功能域为C端球状域(即脂联素球状域,globular adiponectin,gAdiponectin),具有全长脂联素的功能,在骨骼肌中,其活性远大于全长的脂联素。胰升糖素样肽-1(Glucagon-like Peptide-1,GLP-1)是胰岛α细胞和肠粘膜L细胞分泌的一种激素。GLP-1功能域在N端,可与胰岛β细胞上的GLP-1受体结合,促进胰岛素的释放,此作用具有血糖依赖性。GLP-1的体内半衰期很短,尚不能应用于临床。在体内GLP-1主要经二肽肽酶Ⅳ(Dipeptidyl peptidase-Ⅳ,DPP-Ⅳ)的切割和肾脏滤过而清除。寻找有效的方法对GLP-1进行分子改造获得GLP-1类似肽

The main pathophysiological changes in type 2 diabetes are insulin resistance (Insulin resistance, IR) and insulin secretion defects.
