恒河猴脑选择性超深低温断血流复苏模型的制备

【摘要】:目的:探讨猴脑选择性超深低温断血流复苏模型的制备方法,规范动物模型。方法:4-10岁健康恒河猴10只,动物随机分为三组:四血管(双侧颈总动脉及椎动脉)阻断冷灌注组,两血管(双侧颈总动脉)阻断冷灌注组,两血管(双侧颈总动脉)阻断等温灌注组。切开双侧颈动脉鞘,结扎右侧颈总动脉,右侧颈内动脉远心端插管连接冷灌注系统,右侧颈内静脉远心端及双侧股静脉近心端插管连接超滤复温装置,建立脑局部体外循环。夹闭双侧颈外静脉、左侧颈内动脉和颈内静脉(四血管阻断冷灌注组还要阻断双侧椎动脉),经右颈内动脉远心端输入4.0±0.5℃冷灌注液(两血管阻断等温灌注组输入38.0℃灌注液),同时,自右侧颈内静脉远心端及右侧股静脉近心端回流静脉血,回流液超滤复温后经左侧股静脉近心端回输至体循环,维持脑温在14.3~16℃之间60min。手术前后行头颅MRI检查,手术中持续行脑电图监护并监测生命体征及血流动力学指标变化,术后每周行神经功能缺失评分,脑组织及主要脏器行光镜观察。结果:在10只实验猴中,两血管阻断冷灌注组4只恒河猴术中、术后血流动力学稳定,灌注60min后安全复苏并长期存活,术后头颅MRI检查未见异常,术后脑电图正常,术后神经功能评分无异常,主要器官组织形态未见异常;两血管阻断等温灌注组2只恒河猴及四血管阻断冷灌注组4只恒河猴于灌注60min后均未能安全复苏,全部死亡,主要器官组织形态可见不同程度异常。结论:猴脑在选择性超深低温状念下,阻断双侧颈总动脉血流60min后可安全复苏,断血流及复苏对血流动力学、脑组织及神经功能无明显影响。
