兔心肌缺血与再灌注过程对心电图QRS合值及心肌酶的影响

【摘要】:目的:探讨心肌缺血与再灌注过程心电图QRS合值及心肌酶的变化,同时观察Ⅱ导联ST段、T波变化及心律失常发生。方法:采用分别结扎兔冠状动脉左回旋支10min、20min、30min、40min并再灌60min制备4组缺血再灌注动物模型,另设假手术组,分别于再灌注60min时观察QRS合值,Ⅱ导联ST段、T波及心律失常发生情况,并同时测定丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)、肌酸激酶(CK)及肌酸激酶同工酶(CK-MB)。结果:1)QRS合值变化:缺血40min+再灌注60min组QRS合值较假手术组、结扎10min+再灌注60min组及结扎20min+再灌注60min组明增高(P<0.05);2)T波:各组间T波差别无统计学意义(P>0.05);3)ST段:结扎后26只兔出现ST段抬高,2只ST段压低,2只ST段无改变;再灌注60min,21只兔出现ST段回降>0.2mV,2只ST段再抬高>0.1mv,4只ST段无改变,2只恢复正常,1只反而压低;4)心律失常发生情况:缺血40min+再灌注60min组2只兔分别于再灌注40min及43min因窦性心动过缓死亡,2只兔于再灌注3~5min出现室早;5)心肌酶:缺血40min+再灌注60min组AST及LDH明显增高(P<0.05),缺血再灌注组CK均明显高于假手术组(P<0.05),缺血30min+再灌注60min组与缺血40min+再灌注60min较假手术组CK-MB明显增高(P<0.05);结论:缺血40min+再灌注60min组,LDH及CK-MB均明显增高,QRS合值水平亦增高,提示此变化与缺血再灌注损伤有关,具体机制有待进一步探讨。
