人胎咽、食管肌层中a-SMA、MHC-Ⅰ、MHC-Ⅱ、Pax7、Pax3表达的三维分布的研究

【摘要】: 目的通过组织学、免疫组织化学及免疫印迹等方法对咽、食管肌层进行研究,揭示咽、食管肌层的结构特性及三维分布,为临床提供理论依据,探讨咽、食管肌层发育和功能性胸痛的潜在机制。方法①对10例22周胎儿咽、食管及腓肠肌标本行HE染色和一抗为α-平滑肌肌动蛋白α-Smooth Muscle Actin,α-SMA㖞、快肌骨骼肌重链fast myosin heavy chain,MHC-II㖞、慢肌骨骼肌重链slow myosin heavy chain,MHC-Ⅰ㖞、pax7以及pax3抗体的免疫组化SABC染色和免疫印迹。结果①胎儿咽及食管的组织学连续切片能原位、精确地显示咽段、食管肌纤维的总体构型变化,免疫组化染色切片能清楚地区分咽段、食管连续肌纤维的类型及其变化规律。咽部肌纤维表达Pax7、MHC-I、MHC-II,而不表达α-SMA。a-SMA、MHC-I、MHC-II共存于胎儿食管上、中段。Pax7、MHC-I、MHC-II共定位,并主要表达在外部纵行肌,至食管下段逐渐消失。而a-SMA则在食管上端最内层开始表达,呈环行排列,由上至下逐渐增加,并由内肌层扩展到外肌层。Pax3在咽部和食管均阴性表达。同时选用腓肠肌作为阳性对照标本;结果Pax7、Pax3共定位于腓肠肌,且Pax7的表达强于Pax3。②免疫印迹结果显示Pax7、MHC-I、MHC-II共定位于咽、食管的肌层,至食管下端逐渐消失。α-SMA则在食管上端开始表达,由上至下逐渐增加。 结论①胎儿咽、食管骨骼肌的分化沿着由上至食管下端的顺序分化。②Pax7、MHC-I、MHC-II共存于胎儿咽、食管上、中段,支持了Pax7阳性细胞是咽、食管骨骼肌肌祖细胞的假设。③进一步认识了咽、食管骨骼肌和平滑肌的过渡区。以上结论将为人类咽、食管的发育提供新的资料,它将有助于功能性胸痛的临床诊断,并为球囊扩张实验定位提供进一步的理论依据。
