兔肝外胆道内脏感觉上行传导途径的实验研究

【摘要】: 目的:本实验通过造模观察与内脏痛相关核团的表达,探讨内脏感觉传导途径。方法:取新西兰长耳兔32只,随机分为8组,每组4只。分别为两组假手术组(Ⅰ-A组、Ⅰ-B组)、去胆囊组(Ⅱ组)、去胆囊胆管注射大肠杆菌组(Ⅲ组)、胆囊内注射大肠杆菌组(Ⅳ组)、胆管扩张组(Ⅴ组)、扩张Oddis括约肌组(Ⅵ组)、扩张Oddis括约肌并切断膈神经组(Ⅶ组)。造模后Ⅱ、Ⅲ、Ⅳ组三天将兔处死;Ⅴ、Ⅵ、Ⅶ组2小时将兔处死,取脊髓、脑干以及间脑作连续切片后,采用HE染色光镜定位,运用免疫组化技术显示相关核团,并进行统计学处理。结果:1、去胆囊组(Ⅱ组)NOS在脊髓后角(C_(5-7))和孤束核中表达具有统计学意义(p<0.05);2、去胆囊胆管注射大肠杆菌组(Ⅲ组)NOS在脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、丘脑腹后外侧核、孤束核中表达具有统计学意义(p<0.05);3、胆囊内注射大肠杆菌组(Ⅳ组)NOS在脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、丘脑腹后外侧核、孤束核表达具有统计学意义(p<0.05);4、胆管扩张组(Ⅴ组)C-fos在脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、孤束核、丘脑腹后外侧核表达具有统计学意义(p<0.05);5、扩张Oddis括约肌组(Ⅵ组)C-fos在脊髓后角(C_(5-7))和孤束核中表达具有统计学意义(p<0.05);6、扩张Oddis括约肌并切断膈神经组(Ⅶ组)C-fos在脊髓(C_(5-7))后角表达明显降低不具有统计学意义(p>0.05)。结论:1、胆囊和胆管引起的疼痛均与脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、丘脑腹后外侧核、孤束核有关,推测由交感和迷走神经双重支配2、Oddis括约肌引起的疼痛与孤束核有关,推测由迷走神经传入3、肝外胆道的感觉传入不仅包括交感和副交感神经,还有右膈神经。
