额隐窝的CT三维重建和应用解剖

【摘要】: 目的:利用高速螺旋CT三维重建额隐窝,了解额隐窝应用解剖,为鼻内镜额窦开放术提供术前参考。方法:利用TOSHIBA 16排高速螺旋CT对51例临床资料进行额隐窝的三维重建,用VITRA 2软件观察额隐窝毗邻结构。结果:CT三维重建均可辨认出额窦、额窦内口及额隐窝。结论:额隐窝的形态个人差异很大,取决于其与毗邻结构的解剖关系,其中以鼻丘气房和钩突的作用为大。鼻丘气房可做为额窦手术开放额隐窝的解剖标志。内镜手术时,以前鼻棘为起点,与鼻底平面成央角50—60。、半径100mm左右的扇形区域为手术的相对安全界。术前CT三维重建额隐窝对制定手术计划有重要指导意义。
[Abstract]:Objective: to reconstruct the forehead recess with high speed spiral CT and to understand the applied anatomy of the forehead recess, so as to provide preoperative reference for endoscopic frontalis surgery. Methods: TOSHIBA 16 row high speed spiral CT was used to reconstruct the forehead recess in 51 cases. The adjacent structure of the forehead recess was observed by VITRA 2 software. Results: three-dimensional reconstruction of CT could identify the frontal sinus, the internal orifice of the frontalis sinus and the recess of the forehead. Conclusion: the morphology of the forehead recess is very different, which depends on the anatomical relationship between it and the adjacent structure, in which the nasal colliculus and uncinate process play an important role. The nasal colliculus chamber can be used as an anatomical marker for the opening of forehead recess in forehead sinus surgery. In endoscopic surgery, the former nasal spine was used as the starting point, and the central angle with the nasal floor plane was 50 鈮,
