肺炎患儿血清维生素A、D水平研究

【摘要】:目的分别探讨肺炎患儿体内维生素A(Vitamin A,VA)、维生素D(Vitamin D,VD)状况及其与肺炎和病后反复呼吸道感染的关系。方法选取88例因肺炎于我院呼吸中心住院治疗患儿为研究对象,入院时检测其血清维生素A及维生素D水平,同时收集患儿临床资料及电话随访其病后一年内呼吸道感染情况。(1)比较性别间及不同年龄组间血清维生素A、D水平。(2)以流行病学调查全国儿童血清VA平均水平为参照,分析肺炎患儿体内VA水平。(3)以目前研究报道的我国西部地区儿童血清VD平均水平为参照,分析肺炎患儿体内VD水平。(4)88例肺炎患儿按病情轻重分为重症肺炎组和普通肺炎组,比较两组间血清维生素A、D水平。(5)按体内VA水平高低分为VA缺乏组、可疑亚临床VA缺乏组以及VA正常组,比较三组间病后一年内反复呼吸道感染(Recurrent respiratory tract infection,RRI)发生率。(6)按病后一年内是否发生反复呼吸道感染分为RRI组及非RRI组,比较两组间血清VD水平。结果不同性别及年龄间血清维生素A、D水平均无统计学差异(P0.05);肺炎患儿体内维生素A、D水平均低于人群中儿童平均水平;重症肺炎组血清维生素A、D水平均低于普通肺炎组(P0.05);VA缺乏组病后一年内RRI发生率明显高于VA正常组;病后一年内发生RRI组血清VD水平明显低于未发生RRI组(P0.05)。结论本研究中血清维生素A、D水平与性别、年龄无关;肺炎患儿体内维生素A、D水平普遍低下;体内维生素A、D水平高低与肺炎病情轻重及病后再感染风险相关,维生素A、D水平越低,越易发展为重症,病后再感染风险越高。
[Abstract]:Objective to explore the relationship between Vitamin A (VA) and vitamin A (Vitamin D, VD) in children with pneumonia and their relationship with recurrent respiratory tract infection after pneumonia. Methods a total of 88 children hospitalized for pneumonia in our respiratory center were selected as the subjects. Their serum vitamin A and vitamin D levels were detected at the time of admission, and the clinical data and telephone follow-up of respiratory tract infections during the first year after the disease were collected. (1) the serum levels of vitamin A and D were compared between the sexes and different age groups. (2) the VA level in children with pneumonia was analyzed with the reference of the average level of serum VA in the whole country. (3) according to the average level of VD in children's serum in Western China, the level of VD in children with pneumonia was analyzed. (4) 88 cases of pneumonia were divided into severe pneumonia group and common pneumonia group according to the severity of the disease. The serum levels of vitamin A and D were compared between the two groups. (5) according to the level of VA in the body, they were divided into VA deficient group, suspicious subclinical VA deficiency group and VA normal group. The incidence of recurrent respiratory tract infection (Recurrent respiratory tract infection) was compared between the three groups in one year. (6) according to the incidence of recurrent respiratory infection within one year after the disease, it was divided into RRI group and non RRI group, and the serum level of VD was compared between the two groups. Results there were no statistical differences in the level of serum vitamin A, D, age and gender (P0.05); pneumonia in children with vitamin A and D were lower than the average level of the crowd of children with severe pneumonia group; serum vitamin A and D were lower than the ordinary pneumonia group (P0.05); group VA deficiency disease within one year after the occurrence of RRI the rate of VA was significantly higher than that of the normal group; disease within one year after the occurrence of the serum level of VD in group RRI was significantly lower than non RRI group (P0.05). Conclusion in this study, the serum vitamin A level of D has nothing to do with sex and age; pneumonia in children with vitamin A, vitamin D level is generally low; the level of A, D and pneumonia severity and disease after infection risk of vitamin A, the lower the level of D, the more likely to develop into severe disease after infection. The higher the risk.
