腹内压监测在急腹症患儿诊疗中具有良好的应用价值

【摘要】：为了探讨腹内压监测在急腹症患儿诊疗中的应用价值,本研究对2013年5月至2016年5月收治在承德医学院附属医院小儿外科的150例急腹症患儿的临床资料进行统计分析。结果显示,急腹症组弥漫性腹部疾病患儿的IAP显著高于腹部局部炎症患儿(p0.05);弥漫性腹部疾病、腹部局部炎症患儿的IAP均显著高于对照组(p0.05),IAP(7 cm H_2O,10 cm H_2O)比例(66.7%;54.7%)均显著高于对照组(20.0%;0%)(p0.05),不良反应发生率(10.0%)显著高于对照组(0%)(p0.05),IAP与腹部压痛、反跳痛、肌紧张等腹膜炎刺激症状、WBC均显著相关(p0.05),WBC、IAP值诊断急腹症的灵敏性、特异性、阳性预测值、阴性预测值、准确率一致。因此,腹内压监测在急腹症患儿诊疗中具有良好的应用价值。
[Abstract]:To explore the value of intraabdominal pressure monitoring in the diagnosis and treatment of children with acute abdomen. From May 2013 to May 2016, we analyzed the clinical data of 150 children with acute abdomen treated in pediatric surgery, affiliated Hospital of Chengde Medical College. The IAP of children with diffuse abdominal disease in acute abdomen group was significantly higher than that in local abdominal inflammation group (P 0.05). The IAP of children with diffuse abdominal disease and local abdominal inflammation was significantly higher than that of the control group (P 0.05) and the ratio of 10 cm H _ 2O to 7 cm H _ 2O was 66.7 cm. 54.7%) were significantly higher than that of control group (20.0%). The incidence of adverse reactions was 10.0), which was significantly higher than that in the control group (P 0.05) and abdominal tenderness, rebound pain, muscle tension and other symptoms of peritonitis stimulation. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy rate of WBC in the diagnosis of acute abdomen were significantly correlated. The monitoring of intra-abdominal pressure has a good value in the diagnosis and treatment of children with acute abdomen.
