健康儿童左室收缩期心肌纵向应变及应变率正常参考值的初步分析

【摘要】:目的:应用二维斑点追踪成像技术定量分析健康儿童左室收缩期心肌纵向应变及应变率,并进行不同年龄段正常参考值的初步分析。方法:对341例健康儿童超声心动图进行分析,按年龄分成6组,分别为新生儿组(0~28天,1组)22例;婴儿组(㧐28天~㩳1岁,2组)54例;幼儿组(1~㩳3岁,3组)60例;学龄前组(3~㩳6岁,4组)81例;学龄组(6~㩳10岁,5组)62例;青春期组(10~㩳18岁,6组)62例。采用斑点追踪软件分析左心室心尖三个长轴切面的动态图像,获取左室收缩期18节段心肌纵向应变及应变率值和整体应变值。结果:(1)同一室壁各节段间收缩期纵向应变及应变率的差异均有统计学意义(P0.05),且自基底段向心尖段呈逐渐增加趋势;(2)除后壁基底段和中间段外,余各室壁节段收缩期心肌纵向应变(SL)规律为:SL(1组)SL(2组)SL(3、4、5组)SL(6组),且心肌整体纵向应变在年龄组间的趋势与其一致;(3)18节段收缩期心肌纵向应变与年龄呈正相关,以侧壁心尖段相关性最强(r=0.567,P0.01);与心率呈负相关,以侧壁中间段相关性最强(r=-0.529,P0.01);心肌整体纵向应变与年龄呈正相关(r=0.681,P0.01),与心率呈负相关(r=-0.658,P0.01)。结论:初步分析了不同年龄段健康儿童左室收缩期心肌纵向应变及应变率的正常参考值,为临床早期评价患儿左室局部心肌收缩功能异常和进一步研究儿童心肌力学改变奠定基础。
[Abstract]:Objective: to analyze the longitudinal strain and strain rate of left ventricular systolic myocardium in healthy children by two-dimensional speckle tracing imaging. Methods: 341 healthy children were analyzed by echocardiography and divided into 6 groups according to their age. 28 days? 54 cases in 1 year old group and 2 cases in infant group. 60 cases in 3 years old group and 30 cases in preschool group. 81 cases in 4 groups of 6 years old, 6 cases in school age group, 6 cases in school age group, 6 cases in school age group. 62 cases in 10 years old and 5 groups, 10 cases in puberty group, 10 cases in puberty group, 10 cases in puberty group. The dynamic images of three long axis sections of the left ventricular apex were analyzed by speckle tracing software in 62 patients (18 years old). The longitudinal strain and strain rate of 18 segments of left ventricular systolic phase and the global strain value were obtained. Results: there were significant differences in longitudinal strain and strain rate between different segments of the same ventricular wall (P 0.05), and from the basal segment to the apical segment, there were significant differences in the longitudinal strain and strain rate between the segments of the same ventricular wall. In addition to the base segment and the middle segment of the posterior wall, The rule of myocardial longitudinal strain (SLL) in the rest of ventricular wall segments was as follows: 1 / SL1 group / SL2 group and SL3 / 4 / 5 group and SL3 / 6 group, and the trend of global longitudinal strain of myocardium in age group was consistent with that of age group, and the longitudinal strain of 18 segments of systolic myocardium was positively correlated with age. In the apical segment of the lateral wall, the correlation was the strongest, and the correlation was negative with the heart rate, and the correlation between the apical segment of the lateral wall and the heart rate was negative. In the middle segment of the lateral wall, there was the strongest correlation between r-0.529 and P0.01, the longitudinal strain of myocardium was positively correlated with age, and negatively correlated with heart rate. Conclusion: the normal reference values of longitudinal strain and strain rate of left ventricular systolic phase in healthy children of different ages were preliminarily analyzed. It provides a basis for early clinical evaluation of left ventricular regional myocardial systolic dysfunction and further study of myocardial mechanical changes in children.
