钛制弹性髓内钉治疗儿童胫腓骨骨折

【摘要】：目的探讨钛制弹性髓内钉治疗儿童胫腓骨骨折的手术技巧、临床疗效、适应症、并发症及注意事项，，为儿童胫腓骨骨折提供一种切实可行、行之有效的治疗方法。方法回顾性分析我院2008年1月-2013年11月在C形臂X线机监视下采用AO钛制弹性髓内钉（TEN）治疗儿童胫腓骨骨折患儿59例（男43例，女16例），年龄2～14岁，平均6岁。术后所有病例均进行随访，随访内容包括骨折愈合情况、伤口情况、肢体长度、膝踝关节活动范围以及步态等情况。结果所有患儿随访6～36个月，平均16个月。所有骨折均愈合，无严重并发症发生。术后无1例出现感染、钉尾处红肿疼痛、TEN折断、骨折不愈合、骨骺损伤、下肢各关节功能异常等并发症。根据Flynn评价标准，优49例，良10例，优良率达到100％。结论 TEN固定治疗儿童胫腓骨骨折具有软组织损伤小、骨折端血运破坏少、不良反应少、材质优良、稳定性好、骨折愈合快、住院时间短、并发症少等多种优点，是治疗儿童胫腓骨骨折的理想方法之一。
[Abstract]:Purpose. To explore the operative technique, clinical effect, indications, complications and precautions of titanium elastic intramedullary nail in the treatment of tibia and fibula fracture in children, and to provide a feasible and effective treatment method for tibia and fibula fracture in children. Method. 59 children with tibia and fibula fractures were treated with AO titanium elastic intramedullary nail (TEN) under the supervision of C-arm X-ray machine from January 2008 to November 2013 in our hospital (43 males and 16 females, aged 214 years). The average age was 6 years. All cases were followed up including fracture healing, wound condition, limb length, range of knee and ankle joint movement and gait. Results. All the children were followed up for 6 ~ 36 months with an average of 16 months. All the fractures healed without serious complications. According to Flynn evaluation criteria, 49 cases were excellent, 10 cases were good, and the excellent and good rate was 100%. Conclusion. The treatment of tibia and fibula fracture with TEN has many advantages, such as little soft tissue injury, less blood damage, less adverse reaction, good material quality, good stability, quick fracture healing, short hospital stay, less complications and so on. It is an ideal method for the treatment of tibia and fibula fracture in children.
