经导管封堵对先天性心脏病伴肺动脉高压患者血浆脑钠素的影响

【摘要】:背景与目的:肺动脉高压是一种严重危害人类生命健康的疾病,据保守估计,目前全国大概有1300万名肺动脉高压患者。肺动脉高压分为原发性和继发性两种,先天性心脏病是引起继发性肺动脉高压的常见原因。而经导管封堵是治疗先天性心脏病积极有效途径,减缓或改善高肺血流所致肺血管结构重建和肺动脉高压的形成,对于先天性心脏病患者手术的成功及其预后的改善甚为重要。血浆脑钠素(BNP)与心力衰竭、急性冠脉综合症、原发性高血压等疾病有密切关系,BNP的释放与压力呈相关性,且与肺动脉高压程度相关,可作为特发性肺动脉高压预测预后的独立因子。本文旨在讨论先天性心脏病合并肺动脉高压经药物治疗与经导管行封堵术后血浆BNP浓度变化及对预后的影响。 方法:本研究收集2010年2月-2012年2月我院心内科先天性心脏病患者为研究对象,以多普勒超声检查提示肺动脉收缩压≥40mmHg为肺动脉高压初步诊断标准,排除合并其他器质性心脏病、高血压病、先天性复合畸形、肾功能异常、心功能不全患者,共有20例患者纳入研究,其中男性8例,女性12例,房间隔缺损(ASD)10例,室间隔缺损(VSD)4例,动脉导管未闭(PDA)6例。9名先天性心脏病(CHD)患者按先心病介入治疗常规操作成功实行封堵术,术中测量肺动脉压力,分别于术前、术后第二天,术后一个月抽取静脉血5ml,采用酶联免疫法测定血浆BNP水平,同时比较10例先天性心脏病伴肺动脉高压患者经药物治疗前及一个月后血浆BNP水平变化。并于介入治疗术前及术后一周内行二维超声心动图检查,观察各房室大小、左室后壁(LVPW)、左室短轴缩短率(LVFS)、动脉血氧饱和度(SO2)、6min步行距离试验(6MWT)及肺动脉压力(PAP)的变化。 结果:9例先天性心脏病伴肺动脉高压患者成功接受介入治疗,手术顺利,无并发症,封堵术后即刻经导管测定肺动脉压力下降。介入治疗1个月后LWPW、LVFS有所下降,,但无统计学意义。术后24小时测定血浆BNP水平下降,部分室间隔缺损患者BNP水平较术前上升,术后一个月测定血浆BNP水平明显下降并有统计学意义。药疗患者明显。 结论: 1.先天性心脏病伴肺动脉压高压患者血浆BNP水平均有不同程度增高。 2.经导管介入封堵术后一个月血浆BNP水平较术前明显降低并有统计学意义。经导管封堵术治疗先天性心脏病伴肺动脉高压患者血浆BNP下降水平较经传统药物治疗下降幅度明显,并有统计学意义。 3.部分室间隔缺损伴肺动脉高压患者介入治疗后24小时内并未出现预想中肺动脉压力下降至血BNP浓度降低,反而轻度升高,考虑可能原因为手术过程中反复机械刺激心脏或封堵即刻左心室容量负荷加重有关。
[Abstract]:Background & objective: pulmonary hypertension is a serious disease that endangers human life and health. According to conservative estimates, there are about 13 million patients with pulmonary hypertension in China. Pulmonary hypertension is divided into primary and secondary pulmonary hypertension. Congenital heart disease is a common cause of secondary pulmonary hypertension. Transcatheter closure is a positive and effective way to treat congenital heart disease, slow down or improve pulmonary vascular remodeling and pulmonary hypertension caused by high pulmonary blood flow. It is very important for the success of operation and the improvement of prognosis in patients with congenital heart disease. The plasma brain natriuretic peptide (BNP) is closely related to heart failure, acute coronary syndrome, essential hypertension and other diseases. And related to the degree of pulmonary hypertension, The purpose of this paper is to discuss the changes of plasma BNP concentration and its influence on prognosis after drug therapy and transcatheter closure for patients with congenital heart disease complicated with pulmonary hypertension.
