昆明市晋宁县成人孕前健康素养的现状研究

【摘要】：目的：了解昆明市晋宁县成人孕前健康素养具备情况,探索影响成人孕前健康素养的影响因素,为更好的推进优生促进工程,降低出生缺陷发生率提出政策建议。方法：依托于在云南省开展的“优生健康促进工程”,在目前孕前优生促进工程开展现状的基础上,采用立意抽样从昆明市晋宁县18～60岁的成人中选择500位调查对象,对其进行问卷测评,问卷内容从健康素养的角度,以孕前保健内容为基础,分为孕前健康素养的知识、态度和技能三个维度。对市、县级计生部门的领导和工作人员进行个人访谈,以探索健康素养在优生促进工程开展过程中的切入点和可行性。用Epidata3.0建立数据库,并采用SPSS13.0对数据进行统计分析。结果：调查对象中有261人具备孕前健康素养,具备率为53.08%；知识方面、态度方面和技能方面的具备率分别为55.08%、95.73%和49.39%。晋宁县成人对于孕前检查内容、最佳生育年龄、叶酸增补的剂量和方法掌握较差。单因素分析显示性别、年龄、职业、婚姻状况和月收入的人群,孕前健康素养有显著性差异(P0.05)。Logistic回归分析显示婚姻状况、性别、月平均收入和年龄是孕前健康素养的影响因素,通过偏回归系数的绝对值可以看出,影响强度最大的因素是婚姻状况(OR=1.288),其次是性别(OR=0.563),影响强度最小的是年龄(OR=0.048)。个人访谈显示基层计生部门工作人员普遍希望获得专业孕前健康知识的培训,而提高孕前健康素养对于优生促进工程的开展有促进作用。结论：目前昆明市晋宁县成人孕前健康素养具备水平总体一般,并且存在薄弱环节,孕前健康素养的影响因素有婚姻状况、性别、月平均收入和年龄。在优生促进工作中应以男性、主要生活在城镇的高收入人群和未婚成年人群为重点干预对象,加强孕前健康宣传和健康教育：引入更加现代化的宣传方式；加强对基层计生工作人员的专业知识培训。
[Abstract]:Objective: to understand the status of pre-pregnancy health literacy of adults in Jinning County, Kunming City, and explore the influencing factors of adult pre-pregnancy health literacy, so as to better promote eugenics and reduce the incidence of birth defects.Methods: based on the present situation of eugenic health promotion project carried out in Yunnan province, 500 subjects were selected from 1860 years old adults in Jinning County, Kunming City.From the perspective of health literacy, the questionnaire is divided into three dimensions: knowledge, attitude and skills of pre-pregnancy health literacy based on pre-pregnancy health care content.In order to explore the cut-in point and feasibility of health literacy in the process of promoting eugenics, the leaders and staff of city and county family planning department were interviewed.Epidata3.0 was used to establish database and SPSS13.0 was used to analyze the data.Results: 261 subjects had pre-pregnancy health literacy (53.08%), knowledge, attitude and skill were 55.08% and 49.39%, respectively.In Jinning County, the content of pre-pregnancy examination, the best reproductive age, the dosage and method of folic acid supplementation were poor.Univariate analysis showed that sex, age, occupation, marital status and monthly income had significant differences in pre-pregnancy health literacy. Logistic regression analysis showed that marital status, sex, monthly average income and age were the influencing factors of pre-pregnancy health literacy.Through the absolute value of partial regression coefficient, it can be seen that the most influential factor of the intensity is the marital status, the second is the gender, and the least is the age.Personal interviews showed that the staff of the grass-roots family planning department generally wanted to obtain professional pre-pregnancy health knowledge training, and the improvement of pre-pregnancy health literacy to promote the development of eugenics promotion project.Conclusion: at present, the health literacy of adults before pregnancy in Jinning County of Kunming City is general and weak. The influencing factors of pre-pregnancy health literacy are marital status, sex, average monthly income and age.In the promotion of eugenics, we should focus on men, high-income people living in cities and towns and unmarried adults, and strengthen pre-pregnancy health promotion and health education: introduce more modern publicity methods;Strengthen professional knowledge training for grass-roots family planning workers.
