过敏性紫癜患儿外周血树突状细胞Toll样受体与共刺激分子表达特性的初步观察

【摘要】:目的:观察过敏性紫癜(Henoch—Schonlein Purpura,HSP)患儿外周血中树突状细胞(dendritic cells, DC)表面Toll样受体(TLR)表达状况,探讨TLR表达异常在HSP免疫发病机制中的作用。 方法:随机选择2011年12-2012年7月在青岛大学医学院附属医院住院治疗的HSP患儿20例为研究对象,20例健康体检儿童为对照组。无菌条件下采集外周静脉抗凝血,Ficoll密度梯度离心法分离获得外周血单个核细胞(PBMC),经粒-巨噬细胞集落刺激因子(GM-CSF)和白介素4(IL-4)诱导培养获得DC,采用流式细胞仪(FACS)检测DC表面TLR2、TLR3、TLR4、CD83、CD86的表达。 结果:①HSP组DC表面CD83表达率[(52.39±4.20)%]与对照组[(52.44±3.87)%]比较,差异无显著性意义(t=-0.06,p0.05);而HSP组DC表面CD86表达率[(73.81±4.75)%]明显高于对照组[(60.47±3.30)%],差异有显著性统计学意义;(t=2.33,p0.05)。②HSP组DC表面TLR2和TLR3表达率均明显高于对照组分别为[(40.24±5.95)%VS(17.17±3.34)%,(31.31±2.75)%VS(13.47±2.20)%],差异有显著性(t=2.39,2.60,p0.05),而TLR4表达率两组间无显著性差异[(23.51±5.75)%VS(23.47±2.20)%,t=0.03p0.05]。③HSP组与对照组DC表面TLR2和TLR3表达率与CD86表达率皆呈正相关关系(-=0.536~0.941,P0.05),而TLR4表达率与CD86表达率间无明显相关性。 结论:过敏性紫癜患儿DC表面TLR2、TLR3和共刺激分子CD86表达增强且呈现正相关关系,提示TLRs和共刺激分子表达异常在HSP发病机制中起重要作用,可能通过启动以Th2为优势的异常免疫应答参与HSP的发病。
[Abstract]:Objective: to investigate the expression of Toll like receptor (TLR) on dendritic cells (DCs) in children with Henoch-Schonlein Purpura HSPs, and to explore the role of abnormal expression of TLR in the pathogenesis of HSP.Methods: twenty children with HSP who were hospitalized in the affiliated Hospital of Qingdao University Medical College from 12 to 2012 were randomly selected as the control group.Peripheral blood mononuclear cells (PBMCs) were isolated from peripheral blood mononuclear cells by Ficoll density gradient centrifugation under aseptic conditions, and induced by granulocyte-macrophage colony stimulating factor (GM-CSF) and interleukin-4IL-4 (IL-4).The expression of TLR2, TLR3, TLR4, CD83 and CD86 on DC was measured.Results the expression rate of CD83 on DC was 52.39 卤4.20% in HSPgroup and 52.44 卤3.87% in control group.The expression rate of CD86 on DC surface in HSP group [73.81 卤4.75%] was significantly higher than that in control group [60.47 卤3.30%], and the difference was statistically significant. The TLR2 and TLR3 expression rates on DC surface in HSP group were significantly higher than those in control group [40.24 卤3.34 卤3.34 卤3.34 卤3.34 卤3.34].There was no significant difference in the expression of TLR4 between the two groups [(23.51 卤5.75)%VS(23.47 卤2.20) p0.05] .3There was a positive correlation between the expression rate of TLR2 and TLR3 on DC surface and the expression rate of CD86 in the HSPgroup and the control group, but there was no significant correlation between the TLR4 expression rate and the CD86 expression rate.Conclusion: the expression of TLR2TLR3 and costimulatory molecule CD86 on DC surface of children with Henoch-Schonlein purpura is increased and has a positive correlation, suggesting that the abnormal expression of TLRs and costimulatory molecules may play an important role in the pathogenesis of HSP.It may be involved in the pathogenesis of HSP by initiating an abnormal immune response dominated by Th2.
