一起新生儿科导管相关真菌血流感染暴发事件的调查与控制

【摘要】：目的对医院新生儿科白色念珠菌引起的导管相关血流感染(CRBSI)暴发进行调查,为医院感染的防控提供依据。方法对2016年10月17日-11月10日新生儿科白色念珠菌血流感染患儿进行流行病学调查,对现场进行环境卫生学监测,采集空气、物表、手及各类液体标本共184份,同时采取临床干预措施。结果 14例患儿外周血、静脉导管等处检出白色念珠菌,确诊13例CRBSI患儿;患儿静脉导管正压接头和留置针肝素帽灌洗液、暖箱湿化水、输液泵表面、医务人员手等多个标本监测不合格;通过拔除导管、抗菌药物治疗及防控措施实施后,所有患儿均病愈出院。结论应加强医务人员手卫生、严格无菌操作、重视环境和物表的消毒等,从多方面防控医院感染暴发。
[Abstract]:Objective to investigate the outbreak of catheter-associated bloodstream infection (CRBSII) caused by Candida albicans in hospital and provide evidence for prevention and control of nosocomial infection. Methods from October 17 to November 10, 2016, an epidemiological survey was carried out in pediatric children with blood flow infection of Candida albicans. 184 samples of air, table, hand and liquid were collected. At the same time, clinical intervention measures were taken. Results Candida albicans were detected in peripheral blood and venous catheters in 14 cases, positive pressure joint of venous catheter and heparin cap lavage fluid were detected in 14 cases of CRBSI, and the surface of infusion pump was treated with wet water in warm box. After the extubation of catheter, antimicrobial treatment and prevention and control measures, all the children were cured and discharged. Conclusion it is necessary to strengthen hand hygiene, strict aseptic operation, pay attention to disinfection of environment and surface, and prevent and control nosocomial infection from many aspects.
