先天性心脏病并心力衰竭患儿血浆脑利钠肽变化的临床意义

【摘要】：目的观察先天性心脏病(congenital heart disease,CHD)并心力衰竭(heart failure,HF)患儿血浆脑利钠肽(Brain Natriuretic Peptide,BNP)和c反应蛋白(CRP)水平的变化,分析BNP与HF严重程度的关系及在心功能评估中的价值。方法选择本院2013年1月2014年8月在儿科和心胸外科住院的CHD患儿72例,根据改良ROSS评分的评价标准将其分成4组:无HF组(23例)、轻度HF组(21例)、中度HF组(18例)、重度HF组(10例)。选取同一时期在门诊进行健康体检的24例健康儿童作为对照组。分别通过标准程序抽取各组静脉血2 m L,通过ELISA法测定各个实验组的血浆BNP和CRP水平。通过统计学方法分析血浆BNP和CRP水平与心力衰竭严重程度的相关性。结果心力衰竭严重程度与血浆BNP水平明显正相关(r=0.72,P0.05),血浆BNP水平随着心力衰竭严重程度而升高;比较各实验组发现,中重度HF组血浆BNP水平显著高于轻度HF组(P0.01),轻度HF组血浆BNP水平高于无HF组和健康对照组(P0.05)。结论心力衰竭严重程度越发严重,其患者血浆BNP水平越高,临床上通过检测血浆BNP对评估CHD合并HF患儿病情严重程度及判断预后具有重大的临床意义。
[Abstract]:Objective to observe the changes of plasma brain Natriuretic Peptidine (Natriuretic) and c-reactive protein (CRP) levels in children with congenital heart disease (CHD) and heart failure (HF), and to analyze the relationship between BNP and HF severity and its value in the evaluation of cardiac function. Methods Seventy-two cases of CHD in pediatrics and cardiothoracic surgery in our hospital were divided into 4 groups according to the evaluation criteria of modified ROSS score: no HF group (n = 23), mild HF group (n = 21), moderate HF group (n = 18) and severe HF group (n = 10). Twenty-four healthy children who underwent physical examination in outpatient clinic at the same time were selected as control group. The plasma BNP and CRP levels in each experimental group were measured by ELISA method. The correlation between plasma BNP and CRP levels and the severity of heart failure was analyzed statistically. Results there was a significant positive correlation between the severity of heart failure and the level of plasma BNP, and the level of plasma BNP increased with the severity of heart failure. The plasma BNP level in moderate and severe HF group was significantly higher than that in mild HF group (P 0.01). The plasma BNP level in mild HF group was higher than that in HF free group and healthy control group (P 0.05). Conclusion the severity of heart failure is more serious and the plasma BNP level is higher in patients with heart failure. It is of great clinical significance to evaluate the severity of the disease and to judge the prognosis of children with CHD by detecting plasma BNP in clinic.
