儿童复发性呼吸道乳头状瘤19例临床分析

【摘要】：目的探讨儿童复发性呼吸道乳头状瘤的临床特征及诊治方法。方法回顾性分析19例儿童复发性呼吸道乳头状瘤病患儿的临床资料。结果 19例儿童复发性呼吸道乳头状瘤病中男9例、女10例;初次发病年龄4岁者18例;首诊时有喉梗阻呼吸困难表现的患儿13例;电子纤维喉镜检查见乳头状瘤单发者2例、多发者17例;行喉部CT检查12例,显示喉内软组织密度影9例;使用喉显微切割钻行呼吸道乳头状瘤削切术41例次,术中见病变侵及声带19例次、室带11例次、喉室14例次,肿瘤切除彻底;患儿复发时间为1~14个月;死亡2例。结论儿童复发性呼吸道乳头状瘤的发病年龄多在4岁以下,男、女性别之间没有明显差别;最常见的发病部位为声带、室带、喉室,电子纤维喉镜是该病的主要检查手段,用喉显微切割钻切除是治疗该病的有效方法。
[Abstract]:Objective to investigate the clinical features, diagnosis and treatment of recurrent respiratory papilloma in children. Methods the clinical data of 19 children with recurrent respiratory papillomatosis were retrospectively analyzed. Results among 19 children with recurrent respiratory papillomatosis, 9 were male and 10 female, 18 were at the age of 4, 13 had dyspnea of laryngeal obstruction at the first visit, 2 were singly diagnosed by electronic fiberoptic laryngoscopy. Multiple larynx in 17 cases, laryngeal CT in 12 cases, laryngeal soft tissue density in 9 cases, laryngeal microdissection drilling in 41 cases of respiratory papilloma, intraoperative involvement of vocal cord in 19 cases, ventricular zone in 11 cases, and laryngeal cavity in 14 cases. The tumor was resected completely, the recurrence time was 1 ~ 14 months, and 2 cases died. Conclusion the onset age of recurrent respiratory papilloma in children is below 4 years old, there is no significant difference between male and female, the most common sites are vocal cord, ventricular band, laryngeal ventricle and electronic fiberoptic laryngoscope. Laryngotomy is an effective method for the treatment of the disease.
