推拿针刺治疗小儿腹泻文献临床效应的DME法分析

【摘要】：目的：本课题通过对小儿腹泻文献的整理，按临床推拿、推拿针刺和针刺治疗小儿腹泻分类，进行推拿、推拿针刺和针刺治疗小儿腹泻的取穴与手法特点以及治疗效果的临床效应对比分析，，并按照临床流行病学／DME方法学的原则和方法进行设计，衡量临床推拿针刺治疗小儿腹泻的整体研究状况，从而对其作用机制和临床效应进行评价。方法：通过对相关期刊论文(CNKI)、维普数据库和万方数据库的检索，下载1992-2012年已发表的关于推拿、针刺治疗小儿腹泻的文献，共检索所得822篇，符合纳入标准和排除标准的文献共143篇，依照临床流行病学／DME方法学的原则和方法对143篇文献进行分析评价,按照推拿组、推拿针刺组、针刺组治疗小儿腹泻组别进行分类，其中推拿方法治疗组文献有95篇，推拿针刺方法治疗组文献有22篇，针刺方法治疗组文献有26篇，对其文献按照诊断标准、治疗方法、所选穴位、所用手法、治疗效果等进行统计、归纳、整理、分析，衡量各组别治疗小儿腹泻的对象选择、疾病诊断方法、纳入与排除标准、治疗方法、取穴及手法规律特点、疗效评价、疗效结果、统计学处理等，从而评价临床推拿、推拿针刺、针刺治疗小儿腹泻的作用机理及临床效应。结果：推拿针刺是治疗小儿腹泻较为有效的方法，临床上治疗小儿腹泻最常用的疗法是推拿和针刺，治疗小儿腹泻首选的推拿手法是捏脊疗法，针刺疗法中速刺足三里是广泛使用的方法，推拿针刺治疗小儿腹泻的疗程多为10天以内，推拿组治疗小儿腹泻的疗程短于针刺组，各组治疗有效率均达89%以上。结论：推拿针刺组与推拿组治疗进行比较，结果具有显著性差异（p0.005）），推拿组与针刺组进行比较，结果有显著差异（p0.005），提示推拿针刺组和针刺组其疗效均优于推拿组；推拿针刺组和针刺组进行比较，无显著差异（p0.025），尚不能认为推拿针刺组与针刺组疗效不同，但是从疗程来看推拿针刺的治疗效果优于针刺组；多种治疗方法的结合其效果优于单一疗法。
[Abstract]:Objective: to classify the literature of infantile diarrhea according to clinical massage, massage acupuncture and acupuncture treatment of infantile diarrhea. The characteristics of acupoints and manipulations and the clinical effects of massage acupuncture and acupuncture in treating children with diarrhea were compared and analyzed, and were designed according to the principles and methods of clinical epidemiology / DME methodology. To evaluate the mechanism and clinical effect of clinical massage acupuncture in the treatment of infantile diarrhea. Methods: a total of 822 articles on massage and acupuncture treatment for infantile diarrhea were downloaded from CNKI, Weipu and Wanfang databases, which were published from 1992 to 2012. According to the principles and methods of clinical epidemiology / DME methodology, 143 articles were analyzed and evaluated. According to the classification of massage group, massage acupuncture group, acupuncture group for treatment of infantile diarrhea, 143 articles were classified according to the principles and methods of clinical epidemiology / DME methodology. Among them, there were 95 articles in the massage treatment group, 22 in the massage acupuncture treatment group and 26 in the acupuncture treatment group. The effect of treatment was analyzed, summarized, analyzed and evaluated the target selection, disease diagnosis, inclusion and exclusion criteria, treatment methods, points and manipulation characteristics, curative effect evaluation, curative effect results, and so on. To evaluate the mechanism and clinical effect of clinical massage, massage acupuncture and acupuncture in treating infantile diarrhea. Results: massage acupuncture is an effective method in the treatment of infantile diarrhea. The most commonly used treatment for infantile diarrhea is massage and acupuncture. The first choice for treatment of infantile diarrhea is chiropractic. Zusanli is widely used in acupuncture therapy. The course of treatment for diarrhea in children with massage acupuncture is less than 10 days. The course of treatment in massage group is shorter than that in acupuncture group, and the effective rate of treatment in each group is over 89%. Conclusion: there is significant difference between the massage acupuncture group and the massage group, and the difference between the massage group and the acupuncture group is significant (p 0.005), which indicates that the massage acupuncture group and the acupuncture group have better curative effect than the massage group. There was no significant difference between the massage acupuncture group and the acupuncture group, but the therapeutic effect of the massage acupuncture group was better than that of the acupuncture group, but the therapeutic effect of the massage acupuncture group was better than that of the acupuncture group according to the course of treatment. The combination of multiple treatments is superior to monotherapy.
