一起腺病毒引起的聚集性呼吸道感染的实验室调查

【摘要】:目的对一起幼儿园聚集性发热事件开展病原学调查,为疫情处置提供依据。方法采集部分患者呼吸道样品,应用real-time RT-PCR法筛查,腺病毒核酸阳性样品接种HEp-2细胞系,PCR扩增部分腺病毒Hexon高变区基因片段并测序,确定病毒型别并进行变异分析。结果从10份咽拭子标本中检测到7份腺病毒核酸阳性。经分离培养得到7株人腺病毒毒株。系统进化分析确定均为人腺病毒7型。推导的氨基酸序列分析显示,与近年来国内外流行株相比,分离株的Hexon蛋白高变区无氨基酸位点突变。结论结合病例临床表现以及流行病学调查结果,此事件证实系人腺病毒7型引起的急性呼吸道感染聚集性事件。
[Abstract]:Objective to investigate the etiology of an event of agglomeration fever in kindergarten, and to provide evidence for the treatment of epidemic situation. Methods the respiratory tract samples of some patients were screened by real-time RT-PCR. The Hexon hypervariable region gene fragment of adenovirus was amplified and sequenced in HEp-2 cell line. The type of Hexon gene was determined and the mutation was analyzed. Results 7 cases of adenovirus nucleic acid were detected from 10 pharyngeal swabs. Seven human adenovirus strains were isolated and cultured. Phylogenetic analysis confirmed that all of them were human adenovirus 7. The deduced amino acid sequence analysis showed that there was no amino acid mutation in the high variable region of Hexon protein compared with the prevalent strains at home and abroad in recent years. Conclusion combined with the clinical manifestation and epidemiological investigation, this event is confirmed to be a clustering event of acute respiratory tract infection caused by human adenovirus type 7.
