pH值、乳酸、神经烯醇化酶及泛素羧基末端水解酶L1在早产儿脑损伤中的意义

【摘要】:目的探讨动脉血气pH值、乳酸、神经烯醇化酶(NSE)及泛素羧基末端水解酶L1(UCH-L1)水平在早产儿脑损伤中的意义。方法选择符合早产儿脑损伤诊断标准的45例作为观察组,50例无脑损伤早产儿为对照组,所有病例均在生后第1、7天取动脉血查血气分析,并留取静脉血标本检测NSE和UCH-L1。结果生后第1天和第7天两组间pH值的异常率差异有统计学意义(P0.05),观察组第1天与第7天pH值异常比例比较,差异有统计学意义(P0.05),对照组第1天与第7天pH值异常比例比较,差异无统计学意义(P0.05)。两组第1天乳酸值比较差异有统计学意义(P0.05),两组第7天乳酸值比较差异无统计学意义(P0.05),观察组第1天和第7天乳酸值比较,差异有统计学意义(P0.01)。生后第1天和第7天两组间NSE和UCH-L1差异有统计学意义(P0.01),观察组第1天NSE、UCH-L1和第7天比较,差异有统计学意义(P0.05)。结论检测动脉血气、NSE及UCH-L1能够反映早产儿脑损伤,为早期干预提供理论依据。
[Abstract]:Objective to investigate the significance of arterial blood gas pH, lactic acid, neuroenolase (NSE) and ubiquitin carboxyl terminal hydrolase L1 (UCH-L1) in preterm infants with brain injury. Methods 45 preterm infants who met the diagnostic criteria of brain injury were selected as the control group (n = 50). The arterial blood samples were collected on the 1st day after birth for blood gas analysis, and venous blood samples were taken to detect NSE and UCH-L1. Results there was significant difference in the abnormal pH rate between the two groups on the 1st and 7th day after birth (P0.05), and there was a significant difference between the observation group on the 1st day and the 7th day (P0.05), and on the 1st day and the 7th day in the control group, the ratio of the abnormal pH value on the first day and the seventh day was higher than that on the control group. The difference was not statistically significant (P0.05). The lactate value of the two groups on the first day was significantly different (P0.05), the lactate value of the two groups on the 7th day had no statistical significance (P0.05), the observation group on the first day and the seventh day lactic acid value comparison, the difference was statistically significant (P0.01). There was significant difference in NSE and UCH-L1 between the two groups on the 1st and 7th day after birth (P0.01), and there was significant difference between the observation group on the 1st day and the 7th day (P0.05). Conclusion NSE and UCH-L1 can reflect brain injury in premature infants and provide theoretical basis for early intervention.
