重症肺炎患儿病原菌分布特点及耐药性分析

【摘要】：目的探讨和分析重症肺炎患儿病原菌分布特点及抗菌药物耐药趋势。方法选择2014年1月-2016年12月期间医院收治的218例重症肺炎患儿作为临床研究对象,并分别对重症肺炎患儿的病原菌分布特点及抗菌药物耐药趋势进行统计分析。结果 218例重症肺炎患儿中共检出病原菌281株,以革兰阴性菌为主,共147株占52.31%;以痰液检出为主,共208株占74.02%;主要革兰阴性菌对阿米卡星、亚胺培南、头孢哌酮/舒巴坦、美罗培南均表现出较高的耐药性,耐药率均80.00%,对头孢唑林和头孢噻肟则均表现出较低的耐药性,耐药率均25.00%。结论重症肺炎患儿病原菌分布以革兰阴性菌为主,治疗过程中应根据病原菌的分布特点及耐药趋势科学合理选择抗菌药物。
[Abstract]:Objective to investigate and analyze the distribution of pathogenic bacteria and antimicrobial resistance in children with severe pneumonia. Methods 218 children with severe pneumonia from January 2014 to December 2016 were selected as clinical study objects. The distribution characteristics of pathogenic bacteria and the trend of antimicrobial drug resistance in children with severe pneumonia were analyzed statistically. Results 281 strains of pathogenic bacteria were detected in 218 children with severe pneumonia, 147 were Gram-negative bacteria (52.31), 208 were sputum (74.02%), and the main gram-negative bacteria were amikacin, imipenem, cefoperazone / sulbactam. Both meropenem and cefotaxime showed high drug resistance and resistance rates of 80.00g and 25.00g, respectively, and the resistance to cefazolin and cefotaxime were lower than those of cefazolin and cefotaxime. Conclusion Gram-negative bacteria are the main pathogens in children with severe pneumonia. In the course of treatment, antibiotics should be selected scientifically and reasonably according to the distribution characteristics of pathogenic bacteria and drug resistance trend.
