母乳喂养评估量表的初步修订与应用

【摘要】：目的 1.翻译英文版母乳喂养评估量表(MBFES),并对其进行文化调适,使之适合中国文化特征。 2.测定中文版MBFES量表的心理学特征。 3.了解长沙市三级医院产妇母乳喂养现状及其主要影响因素,从而为临床干预提供理论依据。方法 1.采用Brislin翻译法对英文版MBFES量表进行翻译、回译和文化调适。采用探索性因素分析对量表进行建构。选用Cronbach's α信度、分半信度、重测信度检验量表的信度；选用内容效度、结构效度、校标效度考察量表的效度。 2.运用中文版MBFES量表对湖南省长沙市三级医院的166名产妇进行横断面调查。采用单因素方差分析、t检验和多元线性回归分析长沙市三级医院产妇的母乳喂养现状及影响因素。结果 1.通过项目分析和探索性因子分析,MBFES量表1个条目被删除,修订后的中文版MBFES量表由29个条目3个维度组成。 2.信度检验结果：(1)Cronbach's α系数:MBFES各维度的Cronbach's α系数为0.839-0.919,总量表的Cronbach's α系数为0.952。(2)分半信度：MBFES各维度的分半信度系数在0.849-0.901,总量表的分半信度系数为0.949。(3)重测信度：MBFES量表各维度的重测信度系数在0.712-0.790之间,总量表的重测信度为0.805。(4)量表内部一致性：各条目与维度的相关系数在0.574-0.894之间,各维度间相关系数为0.602-0.712,各维度与量表总分间的相关系数为0.851-0.926。 3.效度检验结果：(1)内容效度：MBFES量表经过5名专家的评估,内容效度指数为0.896。(2)校标效度：MBFES量表与母乳喂养自信心量表总分相关系数为0.49,MBFES量表与艾氏产后抑郁量表总分相关系数为0.42,均有统计学意义(P0.01)。(3)结构效度：探索性因子分析显示MBFES量表的3因子模型条目适当,方差累积贡献率达56.788%。 4.本研究样本的MBFES量表的总分均分为108.65±16.27分,逐步多元线性回归分析结果显示,产妇母乳喂养评估得分主要与产妇年龄、食欲、居住环境、喂养方式、是否做出喂养计划、初乳出现的时间、乳头疼痛、产后乳房按摩、以及预计返回工作岗位的时间有关,可以解释65.9%的变异。结论 1. MBFES量表的信度研究表明,该量表的Cronbach's α系数、分半系数和重测信度均达到测量学的要求。 2.3因子模型很好的解释了MBFES量表的结构,与原作者理论构想拟合较好。 3. MBFES量表可以用来评估中国内地产妇的母乳喂养情况,乳头疼痛程度和喂养方式是影响产妇母乳喂养满意度的主要因素。
[Abstract]:Objective 1. English Breastfeeding Assessment scale (MBFES),) is translated and adapted to Chinese culture. 2. The psychological characteristics of the Chinese version of MBFES scale were measured. 3. To understand the current situation of breast-feeding and its main influencing factors in the third class hospital of Changsha, and to provide theoretical basis for clinical intervention. Method 1. The English version of MBFES was translated, translated and culturally adjusted by Brislin. Exploratory factor analysis was used to construct the scale. Cronbach's 伪 reliability, split-half reliability, test-retest reliability test, content validity, structural validity and calibration validity were used to investigate the validity of the scale. 2. The Chinese version of MBFES scale was used to investigate 166parturients in the third class hospital of Changsha, Hunan Province. Single factor analysis of variance (ANOVA), t test and multivariate linear regression analysis were used to analyze the current situation and influencing factors of breast-feeding of parturient in tertiary hospitals of Changsha. Result 1. Through item analysis and exploratory factor analysis, one item of MBFES scale was deleted, and the revised Chinese version of MBFES scale consisted of 29 items and 3 dimensions. 2. The results of reliability test were as follows: (1) Cronbach's 伪 coefficient: the Cronbach's 伪 coefficient of each dimension of MBFES was 0.839-0.919, The Cronbach's 伪 coefficient of the total table is 0.952. (2) split-half reliability: the split-half reliability coefficient of each dimension of MBFES is 0.849-0.901. The split-half reliability coefficient of the total scale is 0.949. (3) retest reliability: the retest reliability coefficient of each dimension of MBFES scale is between 0.712 and 0.790. The test-retest reliability of the total scale was 0.805. (4) the internal consistency of the scale: the correlation coefficient between each item and dimension was 0.574-0.894, and the correlation coefficient between each dimension was 0.602-0.712. The correlation coefficient between each dimension and the total score of the scale was 0.851-0.926. 3. Results of validity test: (1) content validity: the content validity index of MBFES scale was 0.896. (2) calibration validity: the correlation coefficient between MBFES scale and breast feeding self-confidence scale was 0.49. The correlation coefficient between the MBFES scale and the total score of the Ehrlich postpartum depression scale was 0.42, which had statistical significance (P0.01). (3) structural validity. Exploratory factor analysis showed that the 3-factor model items of the MBFES scale were appropriate. The cumulative contribution rate of variance was 56.788%. 4. The total score of MBFES scale of this study was 108.65 卤16.27. The results of stepwise multiple linear regression analysis showed that the evaluation scores of breast-feeding were mainly related to maternal age, appetite, living environment, feeding mode, whether to make feeding plan or not. The timing of colostrum, nipple pain, postpartum breast massage, and estimated time to return to work could account for 65.9 percent of the variation. Conclusion 1. The reliability of MBFES scale showed that the Cronbach's 伪 coefficient, split-half coefficient and test-retest reliability of the scale met the requirements of measurement. The 2. 3 factor model explained the structure of MBFES well and fitted well with the original author's theory. 3. The MBFES scale can be used to assess the breast feeding status in mainland Chinese women. The degree of nipple pain and feeding style are the main factors affecting the maternal breastfeeding satisfaction.
