151例糖尿病母亲新生儿早期并发症临床分析

【摘要】:目的:分析妊娠合并糖尿病及糖尿病母亲孕期血糖控制情况对新生儿的影响,为临床管理糖尿病母亲新生儿早期并发症提供参考依据。 方法:选取2009年1月~2011年12月就诊于吉林大学白求恩第一医院的11191例患儿中确诊为糖尿病母亲新生儿(infant of diabetic mothers,IDMS)共151例作为观察对象,采取回顾性分析方法,观察IDMS发病率情况及早期并发症发生的情况。将151例IDMS分为4组:1、根据胎龄(gestational age,GA)不同划分为早产组(31w≤GA≤36+6w)73例,足月组(37w≤GA≤40+4w)78例。比较IDMS两组并发症的异同点。2、根据产科提供的151例IMDS母亲的孕期血糖控制情况,分为血糖控制良好组(观察1组)72例和血糖控制不良组(观察2组)79例。然后再比较母亲糖尿病妊娠期血糖控制情况与IDMS早期并发症的关系。 结果显示:1、三年来我院IDMS发病率依次为2009年0.93%,2010年1.15%,,2011年1.90%,其中IDMS早产儿发病率依次为2009年0.93%,2010年1.34%,2011年3.05%,IDMS足月儿发病率依次为2009年0.93%,2010年1.15%,2011年1.90%。2、IDMS早期并发症中,大于胎龄儿84例(55.6%),其中巨大儿51例(33.8%),高胆红素血症57例(37.7%),低血糖症46例(30.5%),新生儿湿肺41例(27.2%),RDS19例(12.6%),产伤17例(11.3%),宫内窘迫16例(10.6%),心肌肥厚13例(8.6%),低钙血症13例(8.6%),窒息10例(6.6%),小于胎龄儿10例(6.6%),先天性畸形10例(6.6%),颅内出血5例(3.3%),红细胞增多症2例(1.3%),低血糖脑病2例(1.3%),血栓1例(0.7%)。3、IDMS足月组大于胎龄儿发病率高于早产组,P0.05,差异具有统计学意义。IDMS早产组小于胎龄儿、RDS发病率与足月组比较差异无统计学意义(P0.05)。4、糖尿病母亲妊娠期血糖控制不良组(观察2组)大于胎龄儿、低血糖症、心肌肥厚、产伤、窒息发病率明显高于血糖控制良好组(观察1组),P0.05,差异有统计学意义;而小于胎龄儿、高胆红素血症、RDS、宫内窘迫、先天性畸形、低钙血症、新生儿湿肺发病率与血糖控制良好组(观察1组)比较差异无统计学意义(P0.05)。 结论:1、三年来,我院收治的IDMS发病率逐年上升,其中以IDMS早产儿发病率逐渐增多占优势,提示早产可能为糖尿病母亲妊娠的不良后果之一。2、151例IDMS早期并发症中,大于胎龄儿发生率最高,其它并发症发生率由高到低依次为:高胆红素血症、低血糖症、新生儿湿肺、RDS、产伤、宫内窘迫、心肌肥厚、低钙血症、窒息、小于胎龄儿、先天性畸形、颅内出血、红细胞增多症、低血糖脑病、血栓。3、IDMS足月组大于胎龄儿发生率明显高于早产组;母亲糖尿病对早产儿小于胎龄儿及足月儿RDS发病率有一定影响。4、母亲妊娠期血糖控制良好可减少IDMS早期并发症发生。
