海口地区小儿呼吸道病原体检测结果分析

【摘要】:目的:对海南省中医院、海口市人民医院住院及门诊的小儿呼吸道感染患者检测9种病原体抗体并进行结果分析,探讨海口地区小儿呼吸道患者9种病原体的流行分布情况,不同年龄组、不同性别、不同季节病原体的检出情况,为海口地区小儿呼吸道疾病预防策略制定、临床诊断及治疗提供病原学实验室依据。 方法:收集2013年1月至2013年12月在海南省中医院、海口市人民医院住院及门诊小儿呼吸道感染患者1144例,按照小儿的年龄分期标准,分为4个年龄组。应用间接免疫荧光法同时对9种病原体行血清IgM抗体联合检测及分析研究,这9种病原体分别为嗜肺军团菌(LP),肺炎支原体(MP),Q热立克次体(COX),肺炎衣原体(CPm),腺病毒(AdV),呼吸道合胞病毒(RSV),甲型流感病毒(INFA)和乙型流感病毒(INFB),副流感病毒(PIVs)。 结果:1.1144例患者血清样本中,检出IgM抗体阳性536例,阳性率为46.85%。单项感染339例,阳性率为29.63%。 2.单项感染排前三位的分别是MP,阳性率为22.5%(258/1144)、INFB,阳性率为4.11%(47/1144)、LP阳性率为2.27%(26/1144)。混合感染197例,以MP+INFB感染为主,占10.23%(117/1144):其次为LP+MP混合感染,阳性率为2.62%(30/1144);第三位是LP+MP+INFB混合感染,阳性率为1.92%(22/1144);其阳性率和构成比均不同(P0.05)。 3.4个年龄组阳性率随年龄增加而增加,学龄前期组感染阳性率高,为73.06%(198/270),其次为幼儿组,阳性率为63.00%(126/200),第三位为婴儿后期组,阳性率为48.93%(183/374),各组阳性率及构成比均不相同(P0.05)。 4.1144个病例中,检出536例病原体抗体呈阳性反应,其中男性326例,阳性率为41.85%,构成比为0.61;女性210例,阳性率为57.53%,男性和女性感染的阳性率和构成比均不同(P0.05)。在男性混合感染中,阳性率排在前三位的分别为MP+INFB(阳性率为7.96%)、LP+MP(阳性率为2.70%)和LP+MP+INFB(1.41%)。在女性混合感染中,阳性率排在前三位的分别为MP+INFB(阳性率为15.07%)、LP+MP+INFB(阳性率为3.01%)和LP+MP(阳性率为2.47%),三种组合的病原体间感染阳性率及构成比均各不同(P0.05),同种病原体在不同性别中感染阳性率各不同(P0.05)。 5.四个季节中,夏季感染阳性率高达51.84%,构成比为0.32;秋季为50.16%,春季为48.34,冬季为31.16%。经卡方检验,与冬季相比,其他三个季节阳性率和构成比均显著上升(P0.05),而夏季与秋季间阳性率与构成比均无显著差异(P0.05)。在春季,9种病原体感染阳性率前三位分别为MP、MP+INFB、MP+LP。在夏季,9种病原体感染阳性率前三位分别为MP、MP+INFB、INFB。在秋季,9种病原体感染阳性率前三位分别为MP、MP+INFB、INFB。在冬季,9种病原体感染阳性率前三位分别为MP、LP、INFB。选择感染阳性率较高的4种感染类型,分别为MP、LP、INFB、MP+INFB,分析其在各季节中阳性率差异。MP感染的阳性率在夏季显著上升(P0.05),LP感染的阳性率在冬季显著上升(P0.05),INFB感染的阳性率在秋季显著上升(P0.05),MP+INFB感染的阳性率在秋季显著上升(P0.05)。 结论:1.海口地区9种小儿呼吸道病原体,单项感染以肺炎支原体感染为常见,混合感染以肺炎支原体合并乙型流感病毒感染为主,单项感染多于混合感染。 2.9种小儿呼吸道病原体感染在不同年龄段的阳性率有显著不同,学龄前期组最高,婴儿前期组最低。 3.男性和女性感染的阳性率和构成比均不同。 4.9种小儿呼吸道病原体流行还与季节有关。 5.该研究为海口地区预防策略制定、临床诊断、合理用药提供病原学实验室依据。
[Abstract]:Objective: To study the prevalence and distribution of 9 pathogens in children with respiratory tract infection in the hospital of Hainan, the hospital of the people's hospital in Haikou, and in the clinic, and to study the distribution of 9 pathogens in the children's respiratory tract of Haikou. The detection of pathogens in different seasons provides a basis for the development, clinical diagnosis and treatment of children's respiratory diseases in Haikou.
